Brucella prosthetic valve endocarditis: a systematic review

Abstract: Objective Brucella prosthetic valve endocarditis is a rare but a life-threatening complication of brucellosis. It remains a diagnostic challenge. Optimal treatment of Brucella prosthetic valve endocarditis is debated. Available data is limited to case reports or small case series. The purpose of this study was to systematically review all published cases of Brucella prosthetic valve endocarditis in the literature. … Show more

“…With gastrointestinal involvement, hepatic abscess, granuloma, and peritonitis are observed, while cough, dyspnea, and pleurisy accompany lung involvement, and meningitis, encephalitis, myelitis, and brain abscess develop with neural involvement (18). Although death from Brucellosis is rare, the most common cause of death is cardiovascular system involvement (19). In our study, 31.5% of the patients had organ involvement.…”

“…So, the diagnosis of brucella prosthetic valve endocarditis and its complications is mainly echocardiographic. Despite occasional recovery with antibiotics alone, adding surgery to medical treatment is the most efficient therapeutic approach, yielding significant decrease in mortality [55].…”

“…In Brucella endocarditis, the aortic valve is most frequently impacted, whereas the mitral valve is infrequently affected 8 . The best course of action for treating Brucella endocarditis is still up for debate, although the most generally used method is a mix of medicinal and surgical therapy 9 . While it has been mostly suggested to remove the infected valves and replace them in addition to aggressive antibiotic treatment, it has been reported that only antibiotic treatment might also be adequate 10 .…”
