Objective: Pain is an inevitable symptom in Brucellosis, which causes a wide clinical spectrum. In this study, we aimed to examine the effect of pain on presence, severity, localization, analgesic intake and quality of life in patients diagnosed with Brucella. Materail and Methods: Patients diagnosed with Brucella based on clinical and laboratory findings were included in the study. Age, gender, clinical stage of Brucella, organ involvement, serum C-reactive protein (CRP) level and standard tube agglutination test (STA) titer of the patients were recorded. The Brief Pain Inventory (BPI) and the Turkish Version of the World Health Organization Quality of Life Scale Short Form (WHOQOL-BREF-TR) were administered face-to-face to the patients. Results: The median age of 92 Brucella patients included in the study was 40.5 years. 54.3% of the patients were male and 45.7% were female. At admission, 57.6% were acute, 31.5% subacute, and 10.9% chronic. Organ involvement was present in 31.5% of the patients. At the time of diagnosis, 89.1% of the patients had pain complaints. Pain localization was most frequently in the knee, back, hip and shoulder, respectively. 51.1% of the patients were using nonsteroidal anti-inflammatory drugs (NSAIDs) at least once a day. There was no statistically significant correlation between age, disease stage, organ involvement, serum CRP level and STA titer and WHOQOL-BREF and VAS measurement values. WHOQOL-BREF Psychological (p=0.003) and WHOQOL-BREF Social (p=0.008) measurement values were found to be higher in women than men. Discussion and Conclusion : Pain is a common symptom in Brucella patients. Regardless of age, laboratory parameters and organ involvement, pain in Brucella patients should be questioned at all clinical stages. It affects the quality of life and may cause undesirable side effects by causing frequent painkiller intake.

Brucellosis, one of the common zoonotic diseases all over the world, is still underdiagnosed. Under diagnosis is mostly due to certain concepts that should be revised especially in endemic areas. First of all, in endemic areas especially raw milk consuming communities, it is not logic to consider brucellosis only in certain occupations, while the whole community is exposed through utilization of non-pasteurized dairy products. Unawareness about the protean manifestations of brucellosis and its ability to affect almost every system in the body from heal to crown is another problem that needs orientation. Brucellosis now is one of the great imitators like SLE and TB. Chronic brucellosis can present by different manifestations even in the absence of fever and with low serum antibody titers. So, physicians in endemic areas should be aware by the usual and unusual presentations of brucellosis and should consider brucellosis in the differential diagnosis of almost every unexplained medical problem until proved otherwise.

A combination of antibiotic therapy and cardiac surgery were performed, it seems this approach reduces mortality and complications.AbstractBrucellosis (also known as “undulant fever,” “Mediterranean fever,” or “Malta fever”) is a zoonotic infection transmitted to humans from infected animals (cattle, sheep, goats, camels, pigs, or other animals) by ingestion of food products (such as unpasteurized dairy products) or by contact with tissue or fluids. It is the most frequent zoonosis globally and a major public health issue in many resource‐poor nations. Endocarditis is one of the rarest and most dangerous consequences of brucellosis. Additionally, the combination of endocarditis with cardiomyopathy increases its rarity. This condition is usually treated with a high level of suspicion Serological, clinical, and epidemiological data can all be used to make a diagnosis. The use of echocardiography aids in the early diagnosis. Due to the high risk of recurrence and the extent of tissue destruction brought on by Brucella, the majority of experts advise an early surgical intervention; nevertheless, other writers assert that low‐risk patients also require cautious therapy. In this article, we discuss the situation of a patient who underwent surgery and had Brucella endocarditis and heart failure. In conclusion, a combination of antibiotic therapy and cardiac surgery, reduces mortality and complications associated with Brucella endocarditis and improves patient quality of life.
