Bronchoscopic diagnosis of sarcoidosis

Abstract: Transbronchial lung biopsy (TBLB), transbronchial needle aspiration (TBNA) of mediastinal lymph nodes and bronchoalveolar lavage (BAL) are routinely performed at fibreoptic bronchoscopy. Up to the present time, no data have been available on the efficacy of performing all three of these procedures simultaneously in the bronchoscopic work-up of sarcoidosis. A prospective study was undertaken to compare the diagnostic yield from TBLB, TBNA and BAL in patients presenting with clinical and radiological features ty… Show more

“…The most plausible explanation for this discrepancy is that we undertook between eight and 10 biopsies in each patient in order to maximise diagnostic yield and each sample was only considered adequate if the diameter was o1-2 mm. Most other recent studies took only four to six biopsies [10,19,20,29] or failed to report the number of biopsies in each patient [17]. Four out of five of the patients had minimal, subclinical apical pneumothoraces which did not require chest tube drainage.…”

“…As per Morales [11], TBNA established diagnosis in 53% of patients with stage I sarcoidosis. Leonard et al [12], combined the techniques of TBLB, BAL and TBNA for diagnostic sensitivities of 100% in sarcoidosis.TBNA has also been found useful in the diagnosis of mediastinal mycobacterial lymphadenopathy [13][14][15][16][17]. CT scan guided TBNA in these cases had 83% sensitivity and 100% specificity [18].…”
