Bronchiectasis is associated with delayed diagnosis and adverse outcomes in the New Zealand Common Variable Immunodeficiency Disorders cohort study

Abstract: Summary Common variable immunodeficiency disorders (CVID) are multi‐system disorders where target organ damage is mediated by infective, autoimmune and inflammatory processes. Bronchiectasis is probably the most common disabling complication of CVID. The risk factors for bronchiectasis in CVID patients are incompletely understood. The New Zealand CVID study (NZCS) is a nationwide longitudinal observational study of adults, which commenced in 2006. In this analysis, the prevalence and risk factors for bronchiec… Show more

“…Although regarded as a late-onset immunodeficiency, many patients develop symptoms of CVID in early childhood. 41,42 Most CVID patients have recurrent and severe infections, whereas a substantial minority present with autoimmune and inflammatory sequelae. 43 In nonconsanguineous populations, approximately 25% of CVID patients have a causative genetic defect.…”

“…Similarly, in the recent NZ CVID study, many patients who underwent vaccine challenge responses before SCIG/IVIG treatment had excellent responses to HIB and tetanus toxoid but not to diphtheria toxoid or S pneumoniae. 42,83 It is apparent that there is considerable heterogeneity in responses to vaccines within the spectrum of CVID.…”

“…Reports from developed Asian countries with advanced healthcare systems, such as Taiwan, Japan or South Korea indicate a very low frequency of CVID (16)(17)(18)(19). In contrast, a recent study has shown an unexpected high prevalence of CVID in the indigenous Maōri of New Zealand (20). The basis for these ethnic-specific rates is not known.…”
