Brief Memory and Executive Test: evaluation of a new screening test for cognitive impairment due to small vessel disease

Abstract: the BMET is a simple and quick to administer clinical tool for the detection of VCI in SVD and its differentiation from AD impairment. Further multicentre studies are required to evaluate and compare it with other existing screening tests.

“…Patients were all studied at least 3 months after any stroke to avoid the influence of effects due to acute ischaemia on cognition. Since it was of interest to explore awareness of functioning in patients who showed clear neuropsychological deficit, the group were also were screened for overall neuropsychological impairment; designated as performance being 1.5 SD below the control mean on any four of the eight tests from the Brief Memory and Executive Test Battery (BMET) (Brookes, Hannesdottir, Lawrence, Morris, & Markus, 2012; described next). This created further subgroups of 21 (47%) SVD patients with neuropsychological impairment (mean ( SD ) age 71.1 (9.1) years; 10 males, 11 females) and 24 (53%) without impairment (mean ( SD ) age 68.4 (7.4) years; 15 males, 9 females).…”

“…In this study, we investigated awareness deficits in a well‐phenotyped population of patients with SVD ( n = 45; 21 with defined concomitant neuropsychological impairment) and made comparisons with 24 Alzheimer's disease (AD) patients and a further 80 control participants. Awareness of performance on a range of neuropsychological measures was examined based on the Brief Memory and Executive Test Battery (BMET) (Brookes, Hannesdottir, Lawrence, Morris, & Markus, 2012), exploring the relationship between awareness and memory and executive function. The results revealed significant awareness deficits in both the SVD and AD groups.…”

“…Comparing 6 acute studies 20,21,31,37,38,43 and 6 nonacute studies 22,30,34,39,46,50 ; gave a relative sensitivity, 0.73 (95% CI, 0.58-0.93) and relative specificity, 1.12 (95% CI, 1.01-1.25). These data suggest that accuracy varies with assessment timing and context, with acute testing yielding higher sensitivity and lower specificity.…”
