Breast Reconstruction with the Latissimus Dorsi Flap: Womenʼs Preference for Scar Location

Abstract: Women overwhelmingly prefer the low and middle transverse scar locations. Physicians should consider using these locations primarily in suitable patients, as this may improve overall patient satisfaction following breast reconstruction.

“…In general, studies addressing postsurgical complications in women who underwent latissimus dorsi flap focus on patient satisfaction, scar formation, technical aspects of the surgical procedure, capsular contracture, or problems caused by radiotherapy. [34][35][36][37] Particularly interesting among our findings was the lower prevalence of tissue adhesion in women who underwent latissimus dorsi flap surgery. It is reasonable to infer that filling of the surgical void with the latissimus dorsi flap and silicone gel implants may have contributed to greater tissue mobility, which in turn might have had a positive effect on overall shoulder/arm range of motion.…”

“…Recently, the use of the cutaneous island along the resting tension lines of the skin was described, that is, an island in the inferolateral to superomedial sense, citing that the result is an almost imperceptible scar in the donor area. Bailey [15] (2009) performs a comparative study with the preference of patients between a horizontal and an oblique design, having as using this type of design, Hankins and Friedman [16] report that in order to perform the same it is necessary to release the muscle insertion by 90%, to rotate the pedicle and fix the cutaneous island to its new medial position.…”

“…D'après Bailey et al, cet emplacement cicatriciel recueille le score de satisfaction des patientes le plus élevé en deuxième position derrière la cicatrice transversale basse [7]. Cette voie d'abord médio-dorsale est par ailleurs optimale pour prélever la totalité du muscle dans de bonnes conditions.…”
