Breast Radiotherapy and Early Adverse Cardiac Effects. The Role of Serum Biomarkers and Strain Echocardiography

Abstract: Breast cancer radiotherapy has a clear benefit for both long-term survival and local recurrence rate. However, there is still much concern about the early radiation-induced heart toxicity. This article aimed to clarify the impact of certain cardiac biomarkers and strain echocardiographic imaging on the detection of early cardiac dysfunction. Several studies that reported changes in either echocardiographic and/or serum levels measurements after breast radiotherapy were searched. Despite the established role of… Show more

“…Treatment protocols of patients with primary breast cancer most often include radiotherapy following breast-conserving surgery or mastectomy (2). Radiotherapy for breast cancer can be associated with acute and late toxicities (19)(20)(21). Being scheduled for radiotherapy may cause psychological distress that can lead to significant sleep disorders.…”

“…Melatonin may have protective effects on the heart through direct free radical scavenging and indirectly through its antioxidant properties [90]. The mechanism of melatonin scavenging free radicals seems to be different from most of the other antioxidants because melatonin interacts with ROS without stimulating the redox system; furthermore, melatonin converts free radicals into stable products, which 10 Oxidative Medicine and Cellular Longevity basically do not react with other molecules [26]. In addition, the antioxidant effect of melatonin involves the donation of two electrons, which do not become free radicals after providing electrons to free radicals and neutralizing free radicals to protect cells from free radical attacks.…”

“…The available data show that the higher the radiation dose, the stronger is the cardiotoxicity, and the risk of cardiovascular complications is also increased. Moreover, the risk of cardiovascular complications in patients who received radiotherapy for left breast cancer was significantly higher than the risk in patients who received radiotherapy for right breast cancer [10]. Although the benefit of radiotherapy is obvious, increasing attention has been paid to the cardiac damage induced by radiotherapy, which would suggest limiting the dose and use of radiotherapy in cancer patients [11].…”

“…Increased CRP level was associated with the severity of radiation-induced cardiomyopathy after radiation therapy of lung or breast cancer (18). We have shown elevated levels of inflammatory cytokines such as interleukin (IL)-1, IL-6, and tissue necrosis factor alpha (TNF-α) in serum after local cardiac irradiation in mice (19), but data on their role in the prediction of myocardial changes in clinical trials are lacking to date (20,21).…”
