Breast Milk Iodine and Perchlorate Concentrations in Lactating Boston-Area Women

Abstract: Perchlorate exposure was not significantly correlated with breast milk iodine concentrations. Perchlorate was detectable in infant formula but at lower levels than in breast milk. Forty-seven percent of women sampled may have been providing breast milk with insufficient iodine to meet infants' requirements.

“…Further research is needed to assess the toxicological implications of NIS expression in both placental and mammary tissue (11a) and the modulation of perchlorate transport by iodide. Active transport of perchlorate across membranes is also consistent with the active secretion of perchlorate in human and bovine milk (7,15,19,24).…”

“…It is perhaps not surprising that this association was greater in women with urinary iodine below 100 µg/L, and stronger still among these women who smoke, 90 since cigarettes contain thiocyanates that also inhibit iodine uptake. Because infants are particularly vulnerable to thyroid hormone insufficiency 29 and because perchlorate levels are particularly high in breast milk, 91 it is of concern that perchlorate may be affecting TH signaling in early infant development in some proportion of the US population. 92 Predicting the consequences of environmental perchlorate exposure on thyroid function and thyroid hormone action reveals several serious weaknesses in our understanding of the thyroid system as we employ this information for risk assessment.…”

“…Perchlorate is typically detected in breast milk (Kirk et al, 2005) and thus the data presented here underestimate perchlorate exposure for breast fed infants. Specifically, using the range of mean perchlorate levels in breastmilk extracted from Kirk et al, 2005;Pearce et al, 2007;Kirk et al, 2007, and breastmilk intake estimates compiled by the US EPA (USEPA, 2002. Child-specific Exposure Factors Handbook [interim final].…”
