Breast Intracystic Papillary Carcinoma: An Update

Abstract: Intracystic papillary carcinoma (IPC), a breast tumor mainly occuring in the elderly, has long been considered as a variant of ductal carcinoma in situ (DCIS). This is now debated since metastatic cases have been reported. In this study, surgical pieces of 20 IPCs were reassessed, and markers of myopepithelial layer (p63, CD10 and Smooth Muscle Actin) as well as estrogen receptors (ER) and progesterone receptors (PgR) and C-erb-B2 oncoprotein expression were systematically performed and quantified. In 10 cases… Show more

“…Both, papillary cancers and those with medullary features were subtypes considered as prognostic favorable. [54][55][56][57] Although we cannot explain this finding, it could be possible that papillary and medullary cancers share a particular molecular environment that is nonresponsive to 8p deletion. Our observation that 8p deletions were particularly rare in tubular carcinomas fits well to the low grade and good prognosis of these cancers, however.…”

“…However, recent studies where myoepithelial cells were not detected around papillary carcinoma with DCIS created doubts on the presence of another type of IPC between in situ and invasive forms (8,9). The rate of invasive focus is reported as 40% in many series, although it may vary (2,8,10). The rate of lymph node metastases range between 0-36%, and is much lower than that in normal breast cancer (3).…”

“…Intra cystic papillary carcinoma is a solitary, centrally located malignant papillary proliferation within an encysted or cystically dilated duct. IPC was considered to be a variant subtype of DCIS but a recent review of literature shows its association with DCIS or invasive breast cancer in about 40% cases [5]. In IPC (pure) form, solid papillary tumor is confined within a cystic dilated duct without DCIS or invasion into the surrounding tissue.…”

“…Histologically EPC is characterized by papillary configuration within a well-circumscribed cystic or distended duct with cytological features similar to papillary DCIS. These lesions lack myoepithelial cells at their periphery, in contact to DCIS which exhibits myoepithelial cell at the periphery; recently many authors have considered EPCs as, invasive carcinomas with an expansile growth [4][5][6][7].…”
