Background: Metastasis of breast carcinoma to the paranasal sinus is rare. There is hardly any accumulated data to guide therapy and counseling. Methods and results: We report two cases and performed a systematic review. To date, 40 other cases have been reported since 1939 with a median survival of only 6 months after first presentation. The median time between diagnosis of the primary tumor and discovery of the paranasal metastasis was 33 months. Only nine cases of solitary paranasal sinus metastasis could be identified, having the same poor prognosis. Overall, no indicators could be found for a better prognosis, not even the type of treatment (if any). Conclusions: Paranasal metastases of breast carcinoma are rare but lethal. Treatment should be aimed at palliation only. Extra care should be taken when patients with a history of breast cancer present with complaints of headaches, facial pain, visual impairment or (unilateral) nasal obstruction.

In comparison with cases of benign tumor, a statistically significantly greater number of repeated biopsies were required in cases diagnosed as malignant tumors to confirm the pathological diagnosis or when there was continued suspicion of malignancy. Therefore, when there is clinical suspicion, even if there are no findings of malignancy on the first biopsy, the biopsy should be repeated expeditiously.

The most common malignancy and leading cause of cancer-related deaths in women is breast cancer (BC). Despite significant progress in the management of BC, advanced disease remains an important cause of mortality. 1 Distant metastases are present in 4% of the patients at the time of presentation, with many more subsequently developing distant disease. 2 The most common metastatic sites in advanced BC are bone, lungs, pleura, liver, and brain. Head and neck (H&N) is a rare site for metastases. In most cases, bony involvement of jaws and supraclavicular lymphadenopathy is observed. 3 The metastatic spread to the nasal cavity has been rarely reported, with most cases of renal cell carcinoma. 4-6 BC metastases to the nasal cavity are extremely rare. [7][8][9] Herein, we present a case of nasal septum metastasis from BC. CASE REPORTA 54-year-old female patient presented with a lump in the right breast. She was diagnosed with BC, and a right radical mastectomy and axillary lymph node dissection was performed. The pathology revealed a triple negative invasive ductal carcinoma, T3N0M0. She subsequently received four cycles of adjuvant doxorubicin-cyclophosphamide and 12 cycles of weekly paclitaxel. However, she presented with bilateral pulmonary and bony lesions after 2 years of follow-up. Additionally, she had locoregional recurrence with an ulcerated mass on the surgical site. She had palliative 66 Gy radiotherapy to the anterior wall of the right thorax. She was further treated with palliative 6 cycles of docetaxel, 9 cycles of docetaxel-181 181 181
