Москва, Российская ФедерацияОсновные оперативные вмешательства при смене пола с мужского на женский включают в себя феминизацию грудной клетки (увеличение груди) и генитальную реконструкцию. Приобретение женской формы груди -наиболее значимая часть перехода большинства транс-женщин, эндопротезирование грудных желез чаще выполняется в первую очередь. Следует отметить существенно возросшие требования к эстетическому результату любых полтрансформирующих операций у данной группы пациентов.Цель данного обзора -обобщить современные концепции и стандарты выполнения феминизирующей маммопластики у лиц, желающих сменить пол с мужского на женский. Знание этих аспектов необходимо для достижения наилучшего эстетического результата и удовлетворения пожеланий пациента, обеспечения его психосоциального и физического благополучия. Подробно изучены и представлены такие разделы, как пропорции идеальной груди, развитие груди у транссексуалов под действием гормональной терапии, анатомические различия мужской и женской грудной клетки, размера и положения сосково-ареолярного комплекса, выбор оперативного доступа и расположения имплантата, особенности выбора объема и формы имплантата, формирование и закрепление инфрамаммарной складки, возможные осложнения после аугментационной маммопластики, удовлетворенность результатами операции.Ключевые слова: трансгендер, транссексуал, увеличение груди, сосково-ареолярный комплекс, инфраммарная складка, гормональная терапия, качество жизни, смена пола, секстрансформирующая хирургияThe main surgical operations in the birth-assigned male patient being affirmed as female include feminizing of the chest (breast augmentation) and genital reconstruction. Woman-shape breast formation is an important part of feminization for most trans-women, more often augmentation mammoplasty is performed first. Significantly increased requirements for the aesthetic result of any sex reassignment operations in this group of patients should be noted.The purpose of this review is to summarize modern concepts and standards for augmentation mammoplasty in transgender individuals who want to change their sex from male to female. Knowledge of these aspects is necessary to achieve the best aesthetic result and patients' satisfaction, ensuring their psychosocial and physical well-being. The following sections have been studied and presented in details: proportions of the ideal breast, breast development in transsexuals under the influence of hormonal therapy, anatomical differences between the male and female chest, size and position of the nipple-areola complex, choice of operative access and the implant position, choosing the volume and shape of the implant, formation and fixation of the inframammary fold, possible complications after augmentation mammoplasty, satisfaction with the results of the operation.

Background: Mammoplasty is the most common surgery used for breast augmentation (aesthetic plastic) and breast reconstruction (disease-related plastic) in women who have been diagnosed with and surgically treated for regional breast cancer with modified radical mastectomy. This study aims to examine the long-term effects of mammoplasty on the psychological well-being of women.Methods: Participants were 44 women aged 30–50 years (mean = 40.4 ± 5.9). They were divided into two groups based on the purpose of the breast surgery they underwent [augmentation surgery (AS) vs. reconstruction surgery (RS)] and the time that had elapsed since their surgery (≤3 vs. >3 years).Results: Our findings suggest that women who underwent AS reported a decline in their psychological well-being over time. The women who had undergone AS ≤3 and >3 years did not show any differences in emotional functioning, with the exception of the BREAST-Q scores on the satisfaction with breasts subscale. We examined the impact of mammoplasty on the satisfaction levels and well-being of women who had undergone RS (after MRM). They were less satisfied with their breasts than those who belonged to the AS group, confirming our hypothesis. However, this was true only among those who had undergone surgery ≤3 years earlier.Conclusions: In conclusion, our findings underscore the need to provide psychological support to those who have undergone breast AS and RS. Additionally, this study implies the need for personalized psychological interventions to improve the emotional adaptation process and enhance women's mental well-being.

Objective: Breast augmentation surgery is one of the most common cosmetic procedures among women. In the present study, we compared personality traits, self-esteem, and body perception between women who had undergone breast augmentation surgery and a control group of women who had not. We hypothesized that the personality traits of women who had vs those who had not undergone breast augmentation surgery would differ. Materials and Methods:According to the inclusion and exclusion criteria, patients who had undergone breast augmentation surgery and age-and education-matched, healthy women were included in the present study. The breast augmentation group and control group were compared in terms of personality traits under the Basic Personality Traits Inventory. Additionally, self-esteem, which was assessed with the Rosenberg Self-Esteem Scale, and body perception, which was evaluated using the Body Cathexis Scale, were measured and compared between the two groups.Results: When the patients (n = 80) and the control group (n = 100) were compared, the Body Cathexis Scale, extroversion, and openness scores were statistically significant and were found to be higher in the breast augmentation group (p<0.05). In regression analysis, it was found that age, openness, and the Rosenberg Self-Esteem Scale score had statistically significant effects on extroversion. Conclusion:We argue that there may be a presupposition, based on stigma, that women who undergo breast augmentation surgery are more neurotic than those who do not. Consequently, this may influence the outcomes of studies evaluating the personalities of these women. Our results indicate that women who had undergone breast augmentation had more positive personality traits than women in an un-operated control group.
