Brain Tissue Water Uptake after Middle Cerebral Artery Occlusion Assessed with CT

Abstract: After MCA occlusion, immediate brain tissue net water uptake is associated with a decrease in x-ray attenuation. CT can monitor ischemic edema in an acute stroke.

“…Usually, the ADC drops more than 20% but on CT, the EIE induces a decrease of Hounsfield units of about 0.1% only (referring to the zero point of both scales). Our results indicated a 1.5-HU decrease per percent water increase [25], with good correspondence to animal experiments [120]. EIS on CT are regularly smaller than the corresponding ADC lesion and Somford et al [121] could also prove, that more severe ADC decrease was present in regions with EIS on CT.…”

“…It is noticeable as subtle hypodense changes on CT scans after approximately 45 min in patients [26] -compared to observation of clear diffusion impairment after only 11 min [119]. CT density decrease is directly correlated with water increase [120] and thus represents a tool to monitor the EIE in patients. In contrast to the time course of ADC, CT density decrease is detected later but declines linearly within the first hours [25] thus representing a tissue clock of ischemic tissue damage.…”

“…CT is highly specific for the early identification of ischaemic brain damage [132, 140, 141]. The presence of early signs of ischaemia on CT should not exclude patients from thrombolysis within the first 3 h, though patients with a hypoattenuating ischaemic lesion which exceeds one third of the middle cerebral artery (MCA) territory may benefit less from thrombolysis [126, 134, 135, 142, 143].…”

“…Нативную КТ головного мозга осуществляли в 1-е и 5-е сутки (у 11 человек) посттравматического периода. КТ-перфузию [5] проводили только при поступлении, для нее использовали контрастный препарат ультравист кон-центрацией 300-370 мг/мл объемом 50 мл, который вво-дили болюсно со скоростью 4 мл/с. Выбирали переднюю мозговую артерию, вену Галена, прямой синус либо сток синусов.…”
