Background Brain metastases from differentiated thyroid cancer are uncommon, and the prognosis of patients with them is poor. To explore the optimal treatment for this entity, we retrospectively investigated incidence and features of brain metastases from differentiated thyroid cancer that appeared during and after radioactive iodine therapy. Methods Between 2002 and 2012, 89 patients of differentiated thyroid cancer were included (median age, 63-years; male/female = 29/60). The median follow-up time from first ablation was 63 months (range: 1–175 months). The median cumulative radioactive iodine dose was 8.51 GBq (range: 1.11–42.55 GBq). Results During the follow-up, brain metastases occurred in four (4.5 %) patients. The median follow-up time after first ablation of four patients was 69 months (22, 63, 76 and 105 months). They had already had extracranial metastases at first ablation (lung metastases: 3, lung and bone metastases: 1), and had one or more lesions resistant to radioactive iodine therapy at diagnosis of brain metastasis. Three of them had experienced major hemorrhage from brain metastasis. Survival time intervals from the diagnosis to brain metastasis were 2.5, 3.6, 13 and 13.1 months. Conclusion Brain metastases of differentiated thyroid cancer were relatively uncommon. Prognosis of patients with brain metastasis was unfavorable because of frequent major brain hemorrhage and frequent existence of concomitant extracranial lesions resistant to radioactive iodine therapy. To prevent intracranial hemorrhage, early treatment seemed to be necessary for brain metastases from differentiated thyroid cancer.

Columnar cell variant of papillary thyroid carcinoma (CCV-PTC) is an unusual neoplasm, the clinical behavior of which mainly depends on the encapsulation or infiltration. Patients with extensive extrathyroidal extension usually have an aggressive biological behavior. This study confirmed that beta-human chorionic gonadotropin (β-hCG) secreting invasive CCV-PTC has good prognosis comparing with a cohort of follicular cell differentiated thyroid carcinoma. On the contrary, positive immunoreaction with β-hCG was proved in three anaplastic thyroid carcinoma patients showing aggressive clinical courses. The clinicopathologic characteristics of CCV-PTC and the paraneoplastic syndromes in follicular cell differentiated thyroid carcinoma were further summarized using literature review.

Classically characterized by its papillary appearance, papillary thyroid carcinoma is the most common malignant neoplasm of the thyroid. Several variants of papillary carcinoma have been described considering size (microcarcinoma); nature of the tumor boundaries (encapsulated); architecture (follicular, macrofollicular, cribriform-morular, solid, micropapillary); cellular characteristics (tall cell, columnar, oncocytic, clear cell, hobnail); additional tumor components (papillary carcinoma with focal insular component, papillary carcinoma with spindle and giant cell carcinoma, papillary carcinoma with squamous cell carcinoma, papillary carcinoma with mucoepidermoid carcinoma); stromal features (papillary carcinoma with fasciitis like stroma); or a combination of the abovementioned characteristics (diffuse sclerosing, Warthin-like). Although most variants do not pose any diagnostic problems; some histological variants can be challenging to the pathologist and some are of clinical significance, because of prognostic implications or accompanying clinical conditions. It is not rare to find a papillary carcinoma showing characteristics of more than one variant. For example: an individual tumor can show hobnail features and present as a microcarcinoma; another case can show solid architecture and clear cytoplasm. In such cases, it is more important to comment on clinically relevant morphological characteristics of the tumor, rather than religiously trying to classify an individual tumor into a particular variant. In this review, pathologic and clinical features of papillary carcinoma variants are summarized.
