Solid cell nests (SCN) associated with Hashimoto's thyroiditis may show some atypical nuclear features including prominent nuclear grooves, enlarged overlapping nuclei and nuclear clearing. These features are sometimes mistaken for papillary thyroid microcarcinomas especially when the SCN are numerous. We reviewed SCN associated with Hashimoto's thyroiditis in 12 patients selected from 1,420 archival routinely processed formalin-fixed, paraffin-embedded thyroid specimens of Hashimoto's thyroiditis in which there was more than ten SCN per slide. In addition to the atypical nuclear features, there was a distinct eosinophilic basement membrane surrounding the SCN. Immunohistochemical analysis showed that the SCN were strongly positive for p63, stained weakly for TTF-1 and were negative for thyroglobulin, HBME-1, and calcitonin. This was compared to papillary thyroid microcarcinomas which were strongly positive for thyroglobulin, TTF-1, HBME-1, and variably positive for p63, while calcitonin and chromogranin were negative. These histological and immunophenotypic features can be used to distinguish SCN from papillary thyroid microcarcinomas associated with Hashimoto's thyroiditis.

Концепція, яка основана на висновках, що чинники оточення пухлини різного генезу відіграють важливу роль у її розвитку та метастазуванні, існує давно і тиреоїдні пух-лини, в цьому сенсі, безумовно, не виняток. Водночас свідчення про вплив патологічних змін у позапухлинній тканині щитоподібної залози (ЩЗ) за наявності злоякісної пухлини на характеристики останньої неоднозначні. При цьому вивчення асоціації супутньої тиреоїдної патології та злоякісних пухлин стосувалося і стосується майже виключно папілярної карциноми (ПК) і, в більшості випадків, впливу хронічного тиреоїдиту на її виникнення та формування агресивності [1][2][3][4][5]. Однак і ця інформація часто є супе-речливою, бо за одними даними наявність супутнього тиреоїдиту є сприятливим чин-ником перебігу онкологічного захворювання [6][7][8], за іншими -пухлина на тлі хронічного автоімунного тиреоїдиту проявляє лише окре-мі сприятливі властивості [9] чи біологічні характеристики ПК і перебіг захворювання не залежать від наявності тиреоїдиту, який не має ніякого впливу на прогресування зло-якісної пухлини ЩЗ [5,[10][11][12][13][14][15]. Існують також дані, що ПК може мати гірший клінічний перебіг у певної частини хворих з хронічним тиреоїдитом [16] чи ефект останнього зале-жить від тяжкості його перебігу [17].Незважаючи на широке обговорення пи-тання існування підвищеного ризику виник-нен ня ПК у разі вже існуючого тиреоїдиту (від-носний ризик за деякими даними може сягати 11,8) [18][19][20], а також можливих перехресних механізмів, що призводять до одночасного виникнення обох патологій [2,8,[21][22][23][24][25][26][27][28], тісна кореляція між ПК та хронічним тиреоїдитом до цих пір ще викликає сумнівів [11,29,30]. Немає також точних пояснень щодо супереч-ності між висновками про підвищений ризик виникнення ПК за умов хронічного тиреоїдиту та висновками про сприятливіший прогноз хвороби у таких хворих [4,25].Щодо частоти розповсюдження хроніч-ного тиреоїдиту як супутнього ПК, то нині більшість дослідників вважає її суттєвою, хоча на початку вивчення такої асоціації превалювала інша думка. The BRAF V600E mutation, resulting from the BRAFT1799A transversion, is the most common genetic mutation in papillary thyroid carcinoma (PTC), with a mean frequency close to 50% among all cases. A large number of studies in the past decade have tried to dissect the relevance and the function of the V600E mutation in controlling oncogenesis and progression of thyroid cancer. However, several works published in the latest years have provided new evidence, in partial conflict with the previous knowledge, suggesting the need of reconsidering the meaning of the BRAF V600E mutation in PTC. In this work, we attempt to discuss some of the most recent molecular, preclinical and clinical evidence to construct a more exhaustive model of function for the BRAF V600E in development, progression and therapeutic approach of thyroid cancer.In the past 10 years, a great interest has developed around the biological function of the BRAF V600E mutation in driving cancers. This mutation and its association with different clinical features have been largely investigated in different tumor settings and in particular in melanoma, colon cancer and thyroid carcinoma (Fig. 1). [1][2][3][4][5][6][7][8][9][10] In the field of thyroid oncology, the BRAF mutation is synonymous of malignancy, since benign lesions almost never harbor this mutation, and it is a specific mark of papillary thyroid carcinomas (PTCs), since its occurrence in the follicular histotype (FTCs) is basically missing.11-18 The frequency of this mutation in different types of thyroid tumors is represented in Figure 2. The high specificity of this mutation for the papillary malignant phenotype suggested its clinical application as marker for the detection of PTCs in patients with indeterminate fine needle aspiration biopsies. Indeed, several studies have demonstrated that application of BRAF mutation analysis to cytological samples improves the accuracy of presurgical diagnosis. [19][20][21][22] The high incidence of the BRAF V600E mutation and its association with the malignant phenotype of thyroid nodules has primed researchers to investigate a possible connection between this mutation and aggressive features of papillary cancers. In the past decade, the occurrence of the BRAF V600E mutation has been associated with several morphological and clinical features of PTCs (including extrathyroidal extension, lymph node and distant metastases, higher TNM stage and recurrence), fostering the suggestion that this mutation could be considered a marker of aggressiveness and should be employed in a clinical setting to define high risk profile PTC patients. However, in spite of this large scientific effort, the BRAF V600E mutation still does not take off as clinical prognostic marker.
