Bowen's disease with invasive adnexal carcinoma: The pluripotential nature of Bowen's disease cells

Abstract: Bowen's disease rarely exhibits multiple combinations of premalignant and/or malignant skin lesions. Bowen's disease with invasive adnexal carcinoma was originally described by Kao, but is not well recognized by clinicians due to its rarity and lack of specific clinical features of this condition. Herein, we describe three unusual cases of Bowen's disease with invasive adnexal carcinoma. The two distinct neoplastic areas exhibited continuity both clinically and histologically. The plaque lesions possessed clin… Show more

“…Subsequent hypotheses have included collision tumors, adnexal tumors with secondary BD‐like epidermal involvement (eg, as in bowenoid eccrine porocarcinoma), or neoplasia arising from intraepidermal adnexal structures. Several authors have suggested the possibility of a common origin from a pluripotent epithelial stem cell . In support of this concept, Misago et al recently noted expression of CK19, a marker of stem cells, in a case of BD with trichilemmal differentiation .…”

“…Rarely, cases of BD yielding an invasive adnexal carcinoma have been reported. These include BD in association with sebaceous, trichilemmal, and poroid neoplasia . Even less frequent are reports of BD associated with invasive apocrine/mucinous glandular differentiation .…”

“…Mai et al 21 refer to a pagetoid variant of actinic keratosis and the presence of divergent sweat gland differentiation has been reported in Bowen’s disease 2224. There are, however, no reports thus far describing the presence of acrosyringial or poral differentiation in conventional cutaneous SCCs.…”

“…There were also cases of TLC with primary cutaneous anaplastic large-cell lymphoma, a ductal eccrine component, nevus sebaceous, or a proliferating trichilemmal cyst ( 9 12 ). Some cases were combined with other carcinoma at the lesions, four cases with BCC ( 13 , 14 ), two cases with Bowen’s disease ( 15 , 16 ), and one case with Merkel cell carcinoma ( 17 ).…”
