Bowel dysfunction and irritable bowel syndrome in fibromyalgia patients

Abstract: INTRODUCTION: Irritable bowel syndrome (IBS) is a common public health problem. The condition is characterized by a scarcity of biological markers; thus, diagnostic definitions and classifications have relied to a large extent on symptoms, gastrointestinal manifestations of patients. While thyroid disorder should be considered in the differential diagnosis of patients with IBS symptoms, it is not clear if thyroid disturbances amongst patients with IBS are high enough to warrant routine screening. “…9,10 No previous study has examined the co-occurrence of a comprehensive array of overlapping symptoms and conditions among patients diagnosed as having CFS, FM, or TMD. We hypothesized that, given the similarities between CFS, FM, and TMD, symptoms consistent with overlapping conditions such as IBS and interstitial cystitis and others would likewise be commonly observed.…”

“…Patients with IBS and other chronic inflammatory disorders of the gastrointestinal tract often experience chronic widespread pain (Tobin and Kimmey, 2001). Because endotoxins cause muscle pain, conditions such as fibromyalgia, which frequently co-exist with inflammatory conditions such as Crohn's disease (Sperber et al, 1999), IBS (Sivri et al, 1996;Barton et al, 1999;Sperber et al, 1999) and interstitial cystitis (IC) (Jasmin et al, 1998), may develop secondary to processes that allow increased concentrations of endotoxins into the circulation. It is noteworthy that trauma, surgery and infection, conditions that increase circulating endotoxins, are anecdotal events commonly reported to coincide with the onset of fibromyalgia.…”

“…Several studies conducted for more than a decade underline the benefits of physical training, although some systematic reviews of literature [11,71,72] did not report clearly the effectiveness of this therapeutic option. Such therapy can follow some pain and disability assessments (including various physical tests, pain assessment, and several questionnaires about quality of life, anxiety and depression), which allow for a rigorous longitudinal follow-up [47].…”

“…Patients with FM usually have additional complaints such as pronounced fatigue, sleep disturbances, psychological distress, articular pain, muscular spasms and morning stiffness [91]. The coexistence of FM with other non-musculoskeletal affections, such as genitourinary disorders, irritable bowel syndrome or restless leg syndrome, are frequent [28,72,74,82,85,91,94].…”
