Bovine Acellular Dermal Matrix for Levator Lengthening in Thyroid-Related Upper-Eyelid Retraction

Abstract: BackgroundEyelid retraction is the most common and often the first sign of thyroid eye disease (TED). Upper-eyelid retraction causes both functional and cosmetic problems. In order to correct the position of the upper eyelid, surgery is required. Many procedures have demonstrated good outcomes in mild and moderate cases; however, unpredictable results have been obtained in severe cases. Dryden introduced an upper-eyelid-lengthening procedure, which used scleral grafts, but outcomes were unsatisfactory. A new t… Show more

“… 145 In a study of 32 eyelids in 26 patients, average upper margin reflex distance was lowered from 7.7 mm to 3.3 mm with 69% of patients achieving perfect results. 145 ADMs have also been used to manage nasal lining deficiency in Le Fort 1 osteotomy, prevent Frey syndrome after parotid neoplasm surgery and as an implant for dorsal augmentation in rhinoplasty. 146 148 In patients with cranial defects, ADM has been used to improve bone regeneration and closure of chronic wounds after skull defect reconstruction.…”

“…ADMs have been used in craniofacial surgery to treat soft-tissue defects secondary to congenital conditions, disease and surgical wounds. 142 150 In aplasia cutis congenita (ACC), ADMs have shown utility both as an adjunct to grafting and as a conservative treatment for scalp coverage. 142 , 143 …”

“…Attempts have been made to use biomaterials for clinical posterior lamellar reconstruction. ADM, derived from human (AlloDerm ® , BellaDerm ® ) [ 82 , 109 111 ], porcine (Endurage ® ) [ 112 114 ], or bovine (Surgimen ® ) [ 114 , 115 ] dermis, provides a convenient off-the-shelf alternative for tarsus substitutes due to its good histocompatibility and lower inflammation. They are flexible but sturdy flat sheets of cross-linked collagen matrixes with a basement membrane surface and a dermis surface.…”

“…1,2 Lower eyelid retraction is frequent and can be on of the first signs of TED. 3 Li et al 4 has commented that eyelid retraction is one of the diagnostic criteria TED in Asian patients. Lower lid retraction is defined as the inferior displacement of the lower eyelid margin without eyelid eversion or inversion.…”
