Botulinum neurotoxin in cervical dystonia revisited — recent advances and unanswered questions

Abstract: Cervical dystonia (CD) usually presents a complex pattern of head/neck movements accompanied by tremor, myoclonic jerks and a wide spectrum of non-motor disturbances such as pain, depression, anxiety, and sleep problems. This is the most challenging indication for botulinum neurotoxin (BoNT) treatment. It can offer significant improvement, but it can be difficult after the first injection. Thorough examination and identification of the proper CD pattern, the identification of the muscles responsible, and adjus… Show more

“…Intramuscular botulinum toxin injections (BoNT) are the treatment of choice for motor symptoms in CD. However, BoNT treatment meets only limited patient satisfaction [6]. The therapeutic response becomes apparent within 1-2 weeks after the BoNT injection, with peak effects at approximately 4-6 weeks and a gradual decline thereafter [7,8].…”

Cervical dystonia is associated with high rates of musculoskeletal pain, impaired activities of daily living lower quality of life [ 2 , 3 , 4 , 5 ]. In the last decade, a new clinical classification scheme (the COL-CAP framework) has been utilized when evaluating cervical dystonia [ 6 ].…”

“…This may be caused by several factors including inaccurate diagnosis, primary or secondary resistance to treatment and invalid drug storage. Nevertheless, it seems that appropriate pattern classification, a physician’s experience, BoNT-A dose and precision of injections are the most important factors contributing to successful treatment of CD [ 8 , 9 ].…”
