Zusammenfassung Eine 28-j?hrige Frau entwickelte nach einer Urlaubsreise nach ?gypten erythematosquam?se, pustul?se und eitrig-abszedierende L?sionen am Mons pubis, inguinal und am Unterbauch. Unter dem Verdacht auf eine Dermatomykose wurde topisch antimykotisch behandelt. Die mikrobiologische Diagnostik erbrachte neben Staphylococcus aureus und B-Streptokokken auch den Nachweis eines schnell wachsenden, wei?-gelb pigmentierten, granul?ren Dermatophyten. Die morphologische Artdifferenzierung ergab Trichophyton (T.) mentagrophytes variatio asteroides, einen zoophilen Dermatophyten. Die Behandlung der Kerion Celsi-artigen Dermatomykose im Genitalbereich mit Terbinafin per os erwies sich als langwierig mit nur z?gerlichem Ansprechen. Auch ein kurzzeitger Wechsel auf Fluconazol sowie die antibiotische Therapie mit u.?a. Sultamicillin brachten ebenfalls keinen Fortschritt. Letztlich musste die Patientin mit der Tinea genitalis profunda bis zur Heilung insgesamt ?ber mehr als 4 Monate mit Terbinafin 250?mg behandelt werden. Eine Pseudop?lade Brocq blieb als Restzustand zur?ck. Die Spezies-Identifizierung des Dermatophyten-Isolates mittels Sequenzierung der ITS-Region der rDNA erbrachte f?r den Stamm eine 100?%ige ?bereinstimmung mit beim National Center for Biotechnology Information (NCBI), Bethesda, Maryland, hinterlegten Sequenzen von T. mentagrophytes-St?mmen. Die Variet?t stimmte zudem mit aktuell in Deutschland gefundenen und aus Thailand stammenden zoophilen T. mentagrophytes-Isolaten ?berein. Eine tierische Infektionsquelle war nicht zu eruieren, sodass ?ber eine indirekte ?bertragung wahrscheinlich im Wellness- und Fitnessbereich des Hotels in ?gypten spekuliert werden kann. Als disponierender Faktor muss die auch w?hrend der Behandlung beibehaltene Rasur des Intimbereichs angesehen werden. Inwieweit die neuerdings in Deutschland isolierten zoophilen T. mentagrophytes-St?mme der ?Thailand?-Variante tats?chlich eine h?here Virulenz als die bislang bekannten St?mme dieser Spezies aufweisen, sollte in experimentellen Studien untersucht werden.

Background: Botryomycosis is a rare infectious disease which usually affects the skin. The low virulence of the bacteria tending to form grains and the immune status of the host are important factors in the development of the disease. Methods: We report a case of cervicofacial botryomycosis and review the current literature. Results: A 47-year-old male with a long history of moderate-to-severe atopic dermatitis presented with painful and suppurative nodules of the head and neck. A skin biopsy revealed granules consisting of Gram-positive bacterial colonies in a blossom-like assembly in the center and an eosinophilic rim in the periphery, which are pathognomonic features of botryomycosis. The lesions responded well to systemic antibiotics; however, they rapidly relapsed upon cessation of the treatment. Conclusions: We highlight the well-defined histologic features and recall an almost forgotten disease. We review common predisposing conditions and present evidence that atopic dermatitis might be an additional predisposing factor. © 2014 S. Karger AG, Basel

Botryomycosis is an uncommon, chronic, suppurative, bacterial infection that primarily affects the skin and subcutaneous tissues. It has long been associated with defects of cellular immunity. We report a 28-year-old woman who presented with a chronic, ulcerated lesion with draining sinuses in the right malar region. Predisposing factors were HIV infection with poor immunological control, alcoholism, and a previous trauma to the right cheek. Several courses of antimicrobial therapy provided only partial and temporary remission. Complete clinical remission was only achieved 5 years later when a novel antiretroviral regimen composed of darunavir and raltegravir was initiated.
