Born Too Soon: Care before and between pregnancy to prevent preterm births: from evidence to action

Abstract: Providing care to adolescent girls and women before and between pregnancies improves their own health and wellbeing, as well as pregnancy and newborn outcomes, and can also reduce the rates of preterm birth. This paper has reviewed the evidence based interventions and services for preventing preterm births; reported the findings from research priority exercise; and prescribed actions for taking this call further. Certain factors in the preconception period have been shown to increase the risk for prematurity a… Show more

“…As mentioned earlier, obesity and overweight increase the risk of preeclampsia, gestational diabetes, caesarean delivery, and poor child outcomes, increasing in turn the cost of perinatal care (25), which can be a tremendous burden, particularly in low-and middle-income countries.…”

“…Adolescent pregnancy occurs more frequently in countries in which early marriage is customary, particularly in West Africa, East Africa, and South-Central Asia (24). Earlier marriage (<15 y of age) is associated with greater total parity (24), which in turn is a risk to the nutritional and health status of the mother and her children (25). Pregnancy in adolescence is associated with increased risk of still birth, neonatal mortality, preterm birth, and small size for gestational age compared with in adult mothers (25), and the risk is highest in the youngest adolescents (26).…”

“…The implications for child health and development with respect to an anaemic caregiver have also been recognised (38) . Therefore, prevention strategies should target adolescent girls and women of reproductive age to eliminate or reduce the burden of anaemia before entering into the higher nutritional demands of pregnancy (39)(40)(41) . A continuum in interventions is required during the reproductive period for the benefit to reach the offspring.…”

“…The availability of contraception to avoid unwanted pregnancies and promote optimal IPI is a proposed strategy to reduce the rate of preterm birth 2. However, other independent maternal risk factors for preterm birth may also be risk factors for short IPI 3…”
