Objective: To analyze the capability of a set of neurobiological and psychopathological variables to discriminate bulimia nervosa (BN) patients from healthy controls. Method: Seventy-five female patients with purging BN and 30 healthy controls were compared for psychopathology (impulsivity, borderline personality traits, depressive symptoms and self-defeating personality traits) and neurobiological parameters reflecting hypothalamic-pituitary-adrenal axis activity (morning serum cortisol before and after dexamethasone) and monoamine activity (24-hour urinary excretion of norepinephrine, serotonin, dopamine, and their main metabolites: 3-methoxy-4-hydroxyphenylglycol, 5-hydroxyindoleacetic acid, and homovanillic acid). Furthermore, the relationships between the 2 sets of variables were compared in the 2 samples. Results: BN patients displayed higher impulsivity, more severe depressive features, and more borderline and self-defeating personality traits than controls. The 4 psychopathological variables were strongly interrelated in patients, whereas only depressive features correlated with self-defeating personality traits in controls. Patients had lower 24-hour excretion of serotonin and dopamine than controls, as well as lower ability to suppress cortisol. The relations between the biochemical and the psychopathological variables were only significant in the BN patients, but not in the control group. When discriminant analysis methods were applied, patients and controls differed for psychopathology (impulsive behaviors and borderline personality traits) and biological parameters (baseline cortisol and dopamine excretion), but when the variables were analyzed together, the differences in neurobiological parameters appeared as mediated by the psychopathological status. Discussion: Our results suggest that hypothalamic-pituitary-adrenal axis activity, dopamine activity and other biological parameters are worthy of further study as potential dimensional markers of BN, although they seem to depend on the psychopathological status of the patients, in such a way that the psychopathological items associated with emotional instability (impulsivity and borderline personality traits) seem to be more reliable as clinical markers at the time being.

La phénylcétonurie est une maladie très fréquente (incidence d'environ 1/15000 en population générale) qui fait l'objet d'un dépistage systématique. Transmise sur le mode autosomique récessif, cette affection est due au déficit partiel ou total en phénylalanine hydroxylase, enzyme permettant la transformation de la phénylalanine en tyrosine [1]. Il en résulte une accumulation de phénylalanine dans l'organisme, les concentrations plasmatiques pouvant être 20 à 30 fois plus élevées que la normale. Les études réalisées sur les quotients intellectuels des enfants phénylcétonuriques montrent que les enfants traités dans le premier mois de vie et de façon stricte jusqu'à l'âge de 5 ans ont des performances semblables à celles d'enfants de la population générale, mais infé-rieures d'environ 8 points à celles de leurs germains non atteints [2,3]. Donc, malgré l'instauration du traitement, certains troubles persistent, reflétant une toxicité cérébrale précoce. Cette toxicité a été attribuée à l'excès de phénylalanine dans le système nerveux central, qui inhiberait la synthèse de la myéline et retentirait sur le fonctionnement des neuromédiateurs dont la densité des récep-teurs serait diminuée [4]. Le tableau clinique de la phénylcétonurie présente une variabilité notable selon les individus. Ainsi, certains patients présentent des symptômes tels qu'une hyperactivité, un manque de concentration, une irritabilité ou une anxiété, symptômes dont l'intensité varie en fonction des taux de phénylalanine. Pour un même taux, les enfants peuvent présenter des tableaux cliniques diffé-rents, et la tolérance à des taux élevés peut varier d'un patient à un autre. De même, une diminution du taux de phénylalanine consécutive à un régime appauvri entraîne une amélioration des symptômes chez certains patients, mais pas chez d'autres. Enfin, deux enfants d'une même famille, porteurs de la même mutation, peuvent présenter des tableaux cliniques et cognitifs très différents [5,6]. Cette variabilité clinique suggère l'existence de méca-nismes modificateurs. Un tel mécanisme pourrait-il impliquer un métabolite de la phénylalanine? L'hypothèse présentée ici suggère que la monoamine oxydase de type B (MAOB), enzyme dégradant la phényléthyla-mine -un métabolite de la phénylalanine -pourrait être impliquée dans la genèse des troubles neurologiques précoces et dans la variabilité phénotypique des patients phénylcétonuriques.
