Bony Mallet Thumb

Abstract: Unlike mallet finger, mallet thumb is rare. We treated a case of mallet thumb with avulsion fracture using extension block Kirchner wire technique, and achieved excellent results.

“…Little has been written about mallet fractures of the thumb. 3,9,14 In this study, we compared mallet fractures of the thumb with mallet fractures of other fingers and found that mallet fractures of the thumb generally present for treatment earlier and are not associated with subluxation.…”

“…3,14 There are few reports about mallet thumbs, particularly those with associated fracture. 3,9,14 Little is known about the difference between mallet fractures of the thumb compared with mallet fractures of the index through small fingers with respect to etiology, radiographic findings, and treatment. They are seemingly less common, perhaps due to shorter length and greater size of the thumb.…”

“…The surgical options for bony mallet thumb include tension band fixation, compression fixation pins, and the extension block technique [24]. Teoh and Lee and Theivendran et al have described the use of the hook plate technique for bony mallet finger, but not bony mallet thumb, to obtain anatomical reduction and stable fixation with early mobilization [8, 9].…”

“…Mallet finger is a common sport injury, but bony mallet thumb is rarely encountered because the thumb is shorter than the other fingers and the extensor pollicis longus (EPL) is thicker than the other extensors [1]. It is reported that bony mallet thumb has been managed by tension band fixation, compression fixation pins, or the extension block technique [24]. The anatomical reduction, rigid fixation, early resumption of motion activity, and early return to the work offered by the hook plate technique for bony mallet finger have not previously been reported in bony mallet thumb injuries, so far as we know.…”

“…A series by Robb included mallet deformity of 149 digits, only one of which was a mallet thumb [ 5 ], and a series by Wehbe and Schneider included only one mallet thumb among mallet fractures of 160 digits [ 2 ]. The thumb is shorter than the fingers and the extensor pollicis longus (EPL) is thicker than the extensors of the fingers, which could be one of the reasons for the rarity of EPL avulsion or laceration [ 6 ].…”
