IntroductionChest trauma may be blunt or penetrating and the chest is the third most common trauma region. It is a significant cause of mortality. Multidetector computed tomography (MDCT) has been an increasingly used method to evaluate chest trauma because of its high success in detecting tissue and organ injuries. Herein, we aimed to present MDCT findings in patients with blunt and penetrating chest trauma admitted to our department.MethodsA total of 240 patients admitted to the emergency department of our hospital between April 2012 and July 2013 with a diagnosis of chest trauma who underwent MDCT evaluations were included. Most of the patients were male (83.3%) and victims of a blunt chest trauma. The images were analyzed with respect to the presence of fractures of bony structures, hemothorax, pneumothorax, mediastinal organ injury, and pulmonary and vascular injuries.ResultsMDCT images of the 240 patients yielded a prevalence of 41.7% rib fractures, 11.2% scapular fractures, and 7.5% clavicle fractures. The prevalence of thoracic vertebral fracture was 13.8% and that of sternal fracture was 3.8%. The prevalence of hemothorax, pneumothorax, pneumomediastinum, and subcutaneous emphysema was 34.6%, 62.1%, 9.6%, and 35.4%, respectively. The prevalence of rib, clavicle, and thoracic vertebral fractures and pulmonary contusion was higher in the blunt trauma group, whereas the prevalence of hemothorax, subcutaneous emphysema, diaphragmatic injury, and other vascular lacerations was significantly higher in the penetrating trauma group than in the blunt trauma group (p<0.05).ConclusionMDCT images may yield a high prevalence of fracture of bony structures, soft tissue lacerations, and vascular lesions, which should be well understood by radiologists dealing with trauma.

Chest trauma is classified as blunt or penetrating, with blunt trauma being the cause of most thoracic injuries (90%). The main difference in penetrating trauma is opening the thoracic cavity, created either by stabbing or gunshot wounds, which is absent in blunt chest trauma. Following head and extremities injuries, Blunt thoracic injuries are the third most common injury in polytrauma patients. Assessment of blunt chest trauma patients with clinical and radiographic finding suggestive of pneumothorax, rib fractures, effusion and suspected lung injuries. A cases of 100 patients were included in the study with the age group between 18-49 years. Patients referred from the trauma centre with blunt chest trauma due to road traffic accidents and high velocity trauma. The hospital staff collects data of all trauma patients who were transferred-into the trauma centre with blunt chest trauma. Pneumothorax was detected in 28 patients, 19 patients had associated rib fracture and 11 patients had associated pleural effusion. 11 patients were associated with subcutaneous emphysema .In 19 patients, underlying lung parenchymal injury was present. Hemothorax was detected in 15 patients on CT scan. Pneumomediastinum was detected in 3 patients, while pneumopericardium was not seen in any case. Pulmonary contusion was detected in 15 patients. Computed Tomography (CT) Scan is the modality of choice for rapid assessment of emergency chest trauma patients where associated abdominal injuries can be scanned in one sitting with high sensitivity and specificity.

Мультиспиральная компьютерная томография (МСКТ) более чувствительна, чем рентгенография груди, для выявления легочных, плевральных и костных повреждений у пациентов с политравмой. Высококачественные мультипланарные реконструкции легких привнесли дополнительную информацию к диагностическим возможностям метода.Цель исследования: анализ данных, полученных при проведении МСКТ грудной клетки у детей с политравмой.Материал и методы. Исследовано 147 детей в возрасте от 6 мес до 17 лет с повреждениями груди в структуре политравмы. МСКТ выполнена на томографах Philips Brilliance 16 и 64.Результаты. У исследованных 147 пострадавших основной причиной травмы были дорожно-транспортные происшествия (n = 91 -61,9%), причиной остальных -падение с высоты (56 -38,1%). В 80% случаев травма была тяжелой, сочетанной с повреждением не только легких и грудной клетки, но и других частей тела: голова, живот, таз и конечности. Среди 147 пострадавших у 70% пациентов диагностированы повреждения легочной паренхимы в виде ушибов, реже разрывов; пневмоторакс был обнаружен в 30% случаев, гемоторакс -в 15%.Заключение. МСКТ значительно улучшает обнаружение легочных повреждений при тупой травме груди в связи с высокой информативностью повреждения костных структур, мягких тканей и сосудов, возмож ностью получения мультипланарных и объемных реконструкций изображений. Она позволяет иденти фицировать ушибы легких и дифференцировать участки ателектаза или разрывов, оценивать и рассчитывать их площади и объемы, а поэтому должна всегда использоваться в качестве метода первичной диагностики у пациентов с политравмой.
