Bone Mineral Density in Children and Young Adults with Crohnʼs Disease

Abstract: Reduced bone mineral density (BMD) has been reported in adults with Crohn's disease (CD). Less is known about abnormal BMD in children and young adults with CD. The aims of this study are to determine the prevalence of low BMD and to evaluate the effect of growth and pubertal development on BMD in children and young adults with CD. One hundred-nineteen patients with CD underwent dual-energy X-ray absorptiometry (DXA) to determine BMD. Anthropometry and pubertal development were measured. Bone age was measured … Show more

“…The diminished BMD during flare with significant improvement in remission agrees with previous similar results in IBD patients using either total body or lumbar spine bone mineral density [38-40]. Herzog et al [41] and Ahmed et al [42] reported significant values of 26% and 22% respectively.…”

“…A close relationship between height Z-score and BMD is well documented based on cross-sectional studies in young patients with IBD 39-41 . Although BMD Z-scores measured via dual-energy X-Ray absorptiometry (DXA) are underestimated in kids with low height Z-scores, bone mineralization deficits may persist after adjustments for size.…”

“…Of note is that most of the studies were retrospective. Some investigators found a negative relationship between cumulative glucocorticoid dose and BMD, 395657404158 where others did not 10, 12, 22, 48 . However, glucocorticoids are known to cause osteoblast cell dysfunction and premature apoptosis 596061 , impaired intestinal absorption of calcium 62 and increased urinary calcium excretion 63 .…”

“…IBD is a key risk factor for bone loss and fractures in children 17 as well as adults 813 . Multiple factors are thought to contribute to IBD effects on bone including nutritional status 3,1419 , reduced physical activity/muscle 1,20 , glucocorticoid use 1 and inflammatory cytokines 21,22 .…”
