Bisphosphonate (BP)-related osteonecrosis of the jaw (BRONJ) can occur when enhanced bone-resorptive diseases are treated with nitrogen-containing BPs (N-BPs). Having previously found, in mice, that the non-N-BP etidronate can (i) reduce the inflammatory/necrotic effects of N-BPs by inhibiting their intracellular entry and (ii) antagonize the binding of N-BPs to bone hydroxyapatite, we hypothesized that etidronatereplacement therapy (Eti-RT) might be useful for patients with, or at risk of, BRONJ. In the present study we examined this hypothesis. In each of 25 patients receiving N-BP treatment, the N-BP was discontinued when BRONJ was suspected and/or diagnosed. After consultation with the physician-in-charge and with the patient's informed consent, Eti-RT was instituted in one group according to its standard oral prescription. We retrospectively compared this Eti-RT group (11 patients) with a non-Eti-RT group (14 patients). The Eti-RT group (6 oral N-BP patients and 5 intravenous N-BP patients) and the non-Eti-RT group (5 oral N-BP patients and 9 intravenous N-BP patients)were all stage 2-3 BRONJ. Both in oral and intravenous N-BP patients (particularly in the former patients), Eti-RT promoted or tended to promote the separation and removal of sequestra and thereby promoted the recovery of soft-tissues, allowing them to cover the exposed jawbone. These results suggest that Eti-RT may be an effective choice for BRONJ caused by either oral or intravenous N-BPs and for BRONJ prevention, while retaining a level of anti-bone-resorption. Eti-RT may also be effective at preventing BRONJ in N-BP-treated patients at risk of BRONJ. However, prospective trials are still required.

№ 2/2013 остеопороз и остеопатии оригинальные СтатьиКлиническая практика показала, что эти пре-параты хорошо переносятся пациентами, ред-ко вызывая серьезные побочные эффекты, при этом частота и выраженность нежелательных явлений (Ня) зависела как от дозы и режима введения препарата, так и наличия сопутству-ющих заболеваний [2][3][4]. Среди всех возможных Ня остеонекроз челюсти являлся довольно редким ослож-нением бФ-терапии и в среднем возникал у 6-10% пациентов, получавших их внутривенно по поводу онкологических заболеваний [5]. До настоящего вре-мени почти все сообщения об остеонекрозе касались его локализации в челюстных костях и были связаны с длительным приемом препаратов этого класса [6] . Ис-ключение составляют данные о поражении слуховых костей после хирургического вмешательства и несколь-ких зарегистрированных случаев некроза в бедренных костях. Поэтому есть основания полагать, что челюст-ные кости являются своеобразной «мишенью» для раз-вития этой патологии [7].Остеонекроз челюсти (ОНЧ) на фоне приема бФ определяется как патологическое состояние, характе-ризующееся обнажением некротизированной кости в челюстно-лицевой области, которое продолжается не менее 8 недель [8]. Клинически ОНЧ проявляется появлением дефекта слизистой оболочки с гладкими или неровными краями и обнажением костной ткани (рис.1). ОНЧ может сопровождаться припухлостью, болью, парестезиями, появлением изъязвлений, на-гноением мягких тканей, потерей зубов. При этом на начальных стадиях симптоматика может быть доста-точно скудной. Диагноз ставится при визуальном ос-мотре и на основании клинических данных. Примене-ние лучевых методов существенно расширяет возмож- бФ -это углеродзамещенные аналоги пирофосфатов, имеющие структурное сходство с гидроксиапатитом ко-сти, что делает их способными адсорбироваться на их поверхности и быть устойчивыми к химическому и фермента-тивному гидролизу. бФ длительно сохраняются в кости, пока старая кость не заменится новой в ходе постоянно про-текающих процессов костного ремоделирования. аминобисфосфонаты -это бФ, содержащие в своем составе атом азота (алендронат, ризедронат, ибандронат, памидронат и золедронат), механизм действия этих препаратов связан с ингибицией мевалонатного пути синтеза холестерола и пренилирования ряда белков, необходимых для поддержания структуры и функции остеокластов и предупреждения их апоптоза [1]. алендронат и ризедронат зарегистрированы в нашей стране только для лечения остеопороза (оП), а ибандронат и золендронат также широко применяются и при метастатическом поражении костей, остеолитической гиперкальциемии.
