Objective Identifying patients at high risk for cardiac arrest-associated acute kidney injury (CA-AKI) helps in early preventive interventions. This study aimed to establish and validate a high-risk nomogram for CA-AKI. Methods In this retrospective dataset, 339 patients after cardiac arrest (CA) were enrolled and randomized into a training or testing dataset. The Student’s t -test, non-parametric Mann-Whitney U test, or χ 2 test was used to compare differences between the two groups. Optimal predictors of CA-AKI were determined using the Least Absolute Shrinkage and Selection Operator (LASSO). A nomogram was developed to predict the early onset of CA-AKI. The performance of the nomogram was assessed using metrics such as area under the curve (AUC), calibration curves, decision curve analysis (DCA), and clinical impact curve (CIC). Results In total, 150 patients (44.2%) were diagnosed with CA-AKI. Four independent risk predictors were identified and integrated into the nomogram: chronic kidney disease, albumin level, shock, and heart rate. Receiver operating characteristic (ROC) analyses showed that the nomogram had a good discrimination performance for CA-AKI in the training dataset 0.774 (95%CI, 0.715–0.833) and testing dataset 0.763 (95%CI, 0.670–0.856). The AUC values for the two groups were calculated and compared using the Hanley-McNeil test. No statistically significant differences were observed between the groups. The calibration curve demonstrated good agreement between the predicted outcome and actual observations. Good clinical usefulness was identified using DCA and CIC. Conclusion An easy-to-use nomogram for predicting CA-AKI was established and validated, and the prediction efficiency of the clinical model has reasonable clinical practicability.

The purpose of this study was to determine the association between social and environmental indicators and high mortality rates from chronic kidney disease (CKD) in municipalities of Guatemala. An ecological study of municipal-level factors associated with CKD mortality in Guatemala was conducted. Crude mortality rates were calculated for the 2009–2019 period for each of the country’s 340 municipalities, by gender and age groups. Municipal-level social and environmental indicators were used as independent variables. Linear regression was used for bivariate and multivariate analysis. A total of 28,723 deaths from CKD were documented for the 2009–2019 period. Average crude mortality rate for all ages for the country’s 340 municipalities was 70.66 per 100,000 [0–502.99]. Very highly positive associations with high mortality rates were found in two agrarian territories where land use is mainly for permanent crops (e.g., sugar cane, coffee, rubber, banana, plantain, African palm) and pastures for cattle, with very low percentages of land covered by forests or protected areas. Social factors related to poverty and environmental factors related to agricultural use of land may play a role in the high CKD mortality rates documented in a cluster of municipalities of Guatemala.

Background: Indicators of the metabolic disorder known as metabolic syndrome (MS) include resistance to insulin, poor glucose tolerance, diabetes mellitus (DM), obesity, accumulation of abdominal fat, dyslipidemia, and hypertension. Objective: For evaluation the relationship between intake of vit D and the degree of insulin resistance in MS patients. Patients and Methods: A cross-sectional study was done in Benha University and Teaching Hospital's outpatient clinic. Adult patients with metabolic syndrome who were 18 years of age or older who attended to the outpatient clinic were the subjects of this study. Results: Our findings demonstrated that vitamin D levels of the diabetic and nondiabetic participants were equivalent, but there was a statistically significant relation between DM and HOMA-IR since diabetic patients had considerably higher levels of HOMA-IR than non-diabetics (P<0.001). Conclusion: According to our findings, vit D had a substantial negative correlation with HOMA-IR in both the general population and diabetes patients, with a greater vit D level resulting in a lower HOMA-IR level, as well as a significant negative link with BMI and WC.
