Blount disease

Abstract: Blount disease is an asymmetrical disorder of proximal tibial growth that produces a three-dimensional deformity. Tibia vara is the main component of the deformity. Blount disease exists as two clinical variants, infantile or early-onset, and adolescent or late-onset, defined based on whether the first manifestations develop before or after 10 years of age. The pathophysiological mechanisms are unclear. In the Americas and Caribbean, Blount disease chiefly affects black obese children. Without treatment, the p… Show more

“…In the current study, the gross motor developmental milestones (crawling, sitting, walking) of patients within all three groups of Blount's disease were within normal limits. These findings are corroborated by Inaba et al 11 This contrasts with findings of a review by Janoyer et al 9 The literature regarding sex for both early and late-onset Blount's disease are conflicting. With infantile Blount's disease, some studies found boys and girls to be equally affected while other studies showed either a male or female preponderance.…”

“…[10][11][12] Early onset walking age has been proposed as an association with infantile Blount's disease and has been hypothesised to produce increased stress on the medial aspect of the proximal tibia in the predisposed infant with bow legs. 9 This has been disputed by other work. 11,13 Studies regarding sex and Blount's disease also show a mixture of results.…”

“…Blount’s disease is a pathologic varus deformity that results from disruption of normal cartilage growth at the medial aspect of the proximal tibial physis [ 6 , 7 ]. It may cause severe varus deformity, leg length discrepancy, and articular incongruity [ 7 ].…”

“…But despite all these classi cations, there is still no consensus on treatment. For a long time, it was accepted in early-stage forms that tibial osteotomy before the age of 4 years guarantees a better prognosis, but nowadays, several teams defend the approach of transitory growth modulation by hemiepiphysiodesis in early-stage, leaving osteotomies for neglected forms [4,5]. Even if they all aim at restoring the mechanical axes of the limb, resuming growth, correcting laxity and joint congruence, there are no comparative studies between guided growth (GG) and osteotomy in early-stage of BD hence the interest of this study which aims to compare the results of these two techniques.…”
