Bloodstream Infections in Paediatric Cancer Patients. Prospective Comparative Study in 2 University Hospitals

Abstract: Paediatric cancer treatment centers differ substantially in regard to management of CVADs and in other preventive strategies. The most important use of local surveillance data is longitudinal internal assessment in close cooperation with microbiology and hospital hygiene experts.

“…As expected, higher incidence densities and incidence rates were observed in centre 8 (Table 4), which is a PCC specialised on stem cell transplantation [17,33]. The practice of CVAD care and other strategies to prevent HAIs in paediatric cancer patients differ markedly between the PCCs participating in this study [34]. A survey performed in 2012 including 29 PCCs from Germany, Austria and Switzerland [53] has confirmed this finding.…”

“…The results on blood culture practices impressively show that skilled microbiological methods to defi ne the CVAD as the most probable source of bacteraemia are utilised in scientifi c studies [ 21 ] but not in clinical practice of most paediatric cancer centres [ 37 ] . In clinical practice, routine use of these techniques is hampered by specifi c circumstances in particular by the reluctance to perform a painful peripheral venous puncture in a child in which blood cultures can easily drawn through the CVAD [ 30 ] . Most bloodstream infections (BSIs) in paediatric cancer patients can successfully be managed in situ [ 39 , 42 , 43 ] , but taking into account the heterogeneous answers in the last section of our survey, a word of caution seems to be needed.…”

“…Dieser Beitrag soll zum einen über Studien in der pädiatrischen Onkologie berichten, in denen Präventionsbündel zur Reduktion der Inzidenz von Gefäßkatheter-assoziierten Blutstrominfektionen (CA-BSI) zum Einsatz kamen. Außerdem werden Vorschläge für ein solches Präventionsbündel gemacht, woran kinderonkologische Abteilungen ihr eigenes Vorgehen überprüfen können [58,87].…”
