Blood volume is normal after pre‐operative overnight fasting

Abstract: Our data suggest that even after prolonged pre-operative fasting, cardio-pulmonary healthy patients remain intravascularly normovolaemic. Therefore, hypotension associated with induction of general or neuraxial anaesthesia should perhaps be treated with moderate doses of vasopressors rather than with undifferentiated volume loading.

“…Consequently, the first type of fluid loss is attenuated by slow redistribution between intracellular, interstitial, and intravascular space and causes dehydration, whereas the second type of loss leads to acute hypovolemia. [21] Preoperative hypovolemia after an overnight fasting period, as reported [49,50], cannot be explained by the considerations above and does not occur regularly in all patients [51]. Fluid reloading is unjustified, at least in cardiovascular healthy patients before low-invasive surgery [51].…”

“…It is assumed that the patient had a normal preoperative blood volume, since patients with a healthy cardiopulmonary status generally have an even high normal morning blood volume despite several hours of fasting [9]. Using the measured normal blood volumes per ml 2 of BSA (listed in .…”

“…The concept of loss-adapted fluid and volume therapy is based on the patient's preoperative normovolemia. This is normally present in patients without internal medicine complaints, despite fasting [9]. As mentioned above, under normal conditions, the blood volume of a healthy individual remains physiologically very constant over the course of the entire day; it varies by only a few deciliter.…”

“…Bağırsak tıkanıklığı veya pankreatit gibi bozukluklar interstisyel ödem ve inflamasyon ile volüm kaybına neden olabilir, büyüklüğü hastalık süreci ve resüsitatif çabala-ra bağlıdır. Ciddi kanaması olan hipovolemik hastalarda volüm replasmanından önce cerrahi kanama kontrolü gerekebilir [3,8].…”
