The obstructive sleep apnea (OSA) is highly associated with various significant cardiovascular outcomes such as resistant hypertension (RAH). Despite this, as of now the relationship between high night-time blood pressure (BP) and left ventricular hypertrophy (LVH) in patients with OSA and RAH is not fully understood. The aim of this study was to assess the influence of the addition of eplerenone to a standard antihypertensive therapy on parameters of 24-h ambulatory blood pressure measurement (ABPM) as well as on the results of echocardiography and polysomnography in patients with OSA and RAH. The patients were randomly assigned to one of the two study groups: the treatment group, receiving 50 mg/d eplerenone orally for 6 months (n = 51) and the control group, remaining on their standard antihypertensive therapy (n = 51). After that period, a significant reduction in the night-time BP parameters in the treatment group including an increased night blood pressure fall from 4.6 to 8.9% was noted. Additionally, the number of non-dipper patients was reduced by 45.1%. The treatment group also revealed a decrease in left ventricular hypertrophy and in the apnea–hypopnea index (AHI) with a positive correlation being observed between these two parameters. This study is the first to report the improvement of the circadian BP profile and the improvement of the left ventricle geometry in patients with OSA and RAH following the addition of selective mineralocorticoid receptor antagonists to antihypertensive therapy.

Hypertension is a disease that spreads in the human body caused by increased blood pressure that exceeds normal limits. The increase occurs over a long period, causing complications in human organs that cannot be seen clearly, such as complications in the heart, kidneys, brain, and retina. One of the disorders or complications of high blood pressure is in the retina. The disorder in the retina can also be said as hypertensive retinopathy. Patients suffering from hypertensive retinopathy can only be diagnosed by an ophthalmologist; this is because hypertensive retinopathy cannot be seen with the naked eye. However, one of the earliest signs is the thinning of the arterioles, which can cause blindness. Therefore, computer-assisted processing and analysis of eye fundus images to identify hypertensive retinopathy is an important thing to do by applying the Convolutional Neural Network algorithm. There are nine Convolutional Neural Network architectures used, namely AlexNet, DenseNet, Inception-V3, InceptionResNetV2, Lenet-5, MobileNetV2, ResNet50, VGG16, and VGG19. Based on the experimental results, it was found that of the nine Convolutional Neural Network architectures, two of them, namely AlexNet and Lenet-5, obtained an F1 Measure value of 0.66 and the highest accuracy of 0.67.

Loss of physiological nocturnal blood pressure (BP) decline is an independent predictor of cardiovascular risk and mortality. The aim of the study was to investigate the influence of body composition and fat distribution on 24-hour BP pattern and nocturnal dipping of mean arterial pressure (MAP) in patients with obesity. The study comprised 436 patients, 18 to 65 years old (306 women), with BMI ≥30 kg/m2. Body composition was assessed with dual-energy X-ray absorptiometry (DXA) and blood pressure was assessed by 24-hour BP monitoring. The prevalence of hypertension was 64.5% in patients with BMI <40 kg/m2 and increased to 78.7% in individuals with BMI ≥50 kg/m2 (p = 0.034). The whole-body DXA scans showed that the hypertensive patients were characterized by a greater lean body mass (LBM) and a higher abdominal-fat-to-total-fat-mass ratio (AbdF/FM), while the normotensive participants had greater fat mass, higher body fat percentage and more peripheral fat. Loss of physiological nocturnal MAP decline was diagnosed in 50.2% of the patients. The percentage of non-dippers increased significantly: from 38.2% in patients with BMI <40 kg/m2 to 50.3% in those with BMI 40.0–44.9 kg/m2, 59.0% in patients with BMI 45.0–49.9 kg/m2, 71.4% in those with BMI 50.0–54.9 kg/m2 and 83.3% in patients with BMI ≥55 kg/m2 (p = 0.032, p = 0.003, p<0.001, and p = 0.002 vs. BMI <40 kg/m2, respectively). The multivariable regression analysis showed that patients at the highest quartiles of body weight, BMI, LBM and AbdF/FM had significantly reduced nocturnal MAP dipping compared with patients at the lowest quartiles, respectively.
