To examine serum interleukin 18 (IL-18), fetuin-A, soluble intercellular adhesion molecule-1 (sICAM-1), and endothelin-1 (ET-1) levels in ankylosing spondylitis (AS), psoriatic arthritis (PsA), and Synovitis Acne Pustulosis Hyperostosis Osteitis syndrome (SAPHO). We studied 81 AS, 76 PsA, and 34 SAPHO patients. We measured serum IL-18, fetuin-A, sICAM-1, ET-1, IL-6, IL-23, vascular endothelial growth factor (VEGF), and epidermal growth factor (EGF). IL-18 levels were higher in AS (p = 0.001), PsA (p = 0.0003), and SAPHO (p = 0.01) than in controls, and were positively correlated with CRP (p = 0.03), VEGF (p = 0.03), and total cholesterol (TC, p = 0.006) in AS and with IL-6 (p = 0.03) in PsA. Serum fetuin-A levels were lower in AS (p = 0.001) and PsA (p = 0.001) than in controls, and negatively correlated with C-reactive protein (CRP) in AS (p = 0.04) and SAPHO (p = 0.03). sICAM-1 positively correlated with CRP (p = 0.01), erythrocyte sedimentation rate (ESR, p = 0.01), and IL-6 (p = 0.008) in AS, and with IL-6 (p = 0.001) in SAPHO. Serum ET-1 levels were lower in AS (p = 0.0005) than in controls. ET-1 positively correlated with ESR (p = 0.04) and Disease Activity Score 28 (DAS28, p = 0.003) in PsA. In spondyloarthritis, markers of endothelial function correlated with disease activity and TC.

ObjectiveTo identify an immunologic basis for the male sex bias in ankylosing spondylitis (AS).MethodsCohorts of male and female patients with AS and age‐ and sex‐matched healthy control subjects were selected, and the levels of serum cytokines (interferon‐γ [IFNγ], tumor necrosis factor α, interleukin‐17A [IL‐17A], and IL‐6) were examined by enzyme‐linked immunosorbent assay, the frequencies of Th1 and Th17 cells were assessed by flow cytometry, and whole blood gene expression was analyzed using both microarray and NanoString approaches.ResultsThe frequency of IL‐17A and Th17 cells, both of which are key factors in the inflammatory Th17 axis, was elevated in male patients with AS but not in female patients with AS. In contrast, AS‐associated alterations in the Th1 axis, such as the frequency of IFNγ and Th1 cells in serum, were independent of a patient's sex. Results of microarray analysis supported an altered Th17 axis in male patients, with a specific increase in IL17RA. In addition, male and female patients with AS displayed shared gene expression patterns, while male patients with AS had additional alterations in gene expression that were not seen in female patients with AS. The differential sex‐related immune profiles were independent of HLA–B27 status, clinical disease activity (as measured by the Bath Ankylosing Spondylitis Disease Activity Index), or treatment (with nonsteroidal antiinflammatory drugs or biologic agents), implicating intrinsic sexual dimorphism in AS.ConclusionThe results of this study demonstrate distinct sexual dimorphism in the activation status of the immune system in patients with AS, particularly in the Th17 axis. This dimorphism could underlie sex‐related differences in the clinical features of AS and could provide a rationale for sex‐specific treatment of AS.

Актуальність. Анкілозуючий спондиліт є хронічним прогресуючим системним автоімунним ревматичним захворюванням хребта, периферичних суглобів, крижово-клубових зчленувань, ентезисів, зв’язкового апарату й внутрішніх органів, належить до факторів ризику тяжкої судинної патології. Мета дослідження: вивчити при анкілозуючому спондиліті клініко-інструментальний перебіг ангіопатії, з’ясувати патогенетичні зв’язки з імунними показниками. Матеріали та методи. Під наглядом перебували 79 хворих віком від 15 до 66 років (в середньому 38 років), серед яких чоловіки становили 95 %, жінки — 5 %. Тривалість хвороби від перших її проявів дорівнювала 11 рокам, І ступінь активності відмічався в 32 % спостережень, ІІ — у 39 % і ІІІ — у 29 %, а співвідношення повільнопрогресуючого та швидкопрогресуючого перебігу патологічного процесу становило 4 : 1. Периферичний моноолігоартрит констатовано в 47 % від числа хворих, поліартрит — у 53 %. Пацієнтам виконували ехокардіографію, ультразвукову допплерографію судин й біомікроскопію судин кон’юнктиви. Результати. Ангіопатія спостерігається у 85 % від числа хворих на анкілозуючий спондиліт, розвиток якої прямо пов’язаний зі ступенем активності патологічного процесу й обернено — із серопозитивністю за ревматоїдним фактором. Шкірний васкуліт залежить від віку пацієнтів, увеїт, склерит, синдром Рейно, ураження центральної та периферичної нервової системи — від особливостей перебігу суглобового синдрому. Інтегральні клінічні й інструментальні судинні показники вірогідно співвідносяться з імунними параметрами — рівнями ревматоїдного фактора, С-реактивного протеїну, циркулюючих імунних комплексів, значеннями імуноглобулінемії, а ураження судин перебігає з підвищенням значень легеневого артеріального тиску та судинного опору, у генезі яких бере участь C-реактивний протеїн, що має прогностичну значущість.

Abstract: Our findings suggest a contribution of immune dysregulation to enhanced monocyte-endothelial interactions in AS, especially in patients with active disease, which possibly can accelerate atherogenesis on a longterm basis.
