Blood Aspergillus PCR: The Good, the Bad, and the Ugly

Abstract: Invasive Aspergillosis (IA) is one of the most common invasive fungal diseases and is accompanied by high morbidity and mortality. In order to maximize patient outcomes and survival, early and rapid diagnosis has been shown to be pivotal. Hence, diagnostic tools aiding and improving the diagnostic process are ambitiously searched for. In this context, polymerase chain reaction (PCR) may represent a potential candidate. Its additional value and benefits in diagnosis have been demonstrated and are scientifically… Show more

“…Aspergillus PCR has demonstrated high sensitivity and high negative predictive value in severely immunocompromised patients in settings where antifungal prophylaxis is not used [ 78 , 79 , 80 , 81 ]. PCR is also an important diagnostic test for mucormycosis [ 82 , 83 ].…”

“…PCR is also an important diagnostic test for mucormycosis [ 82 , 83 ]. However, the performance of PCR from blood is impacted significantly in patients receiving mold-active antifungal agents as shown in a recent review [ 8 , 55 , 81 , 84 ]. While mold-active prophylaxis seems to affect Aspergillus PCR on BALF less than blood, reduced diagnostic performance has also been described from BALF [ 85 ].…”
