Bleeding complications after systematic switch of routine thromboprophylaxis for major orthopaedic surgery

Abstract: SummaryThe rate of bleeding complications after major orthopedic surgery approximates 2%. It is unclear whether a systematic switch of routine thromboprophylaxis has an impact on the rate of postoperative bleeding complications. We analyzed prospectively recorded postoperative bleeding complications and symptomatic venous thromboembolic events in 8,176 consecutive orthopedic patients at the Schulthess Clinic Zurich during a systematic switch of thromboprophylaxis from nadroparin to enoxaparin in the year 2004.… Show more

“…In VWD patients who underwent a surgical intervention, we also found that elderly patients reported more post‐surgical bleeding than younger ones, possibly because they had undergone more major surgical procedures, such as orthopedic surgery, which is characterized by a high bleeding risk . Indeed, in our cohort more orthopedic interventions were performed in elderly patients than in younger ones (22/64, 34% vs. 104/495, 21%; P = 0.019).…”

“…In a cohort study by Kistler et al (26), a change of thromboprophylaxis regimen in hip and knee replacement surgery did not impact the rates of bleeding complications. They reported bleeding complications in about 1.6 to 2.5% of patients, but found no significant impact of the type of thromboprophylaxis on VTE or bleeding rates or the rates of surgical complications.…”

“…Medical literature demonstrates a large heterogeneity in the reported rates of major and minor bleeding during extended thromboprophylaxis with LMWH in patients undergoing major orthopedic surgery 1, 2–10, 11–14, 39–41. Freedman et al 1 in their meta‐analysis reported a risk of minor wound‐bleeding and major wound‐bleeding of, respectively, 8.9% and 1.51% following an elective total hip arthroplasty.…”
