Blastomycosis in Minnesota, USA, 1999–2018

Abstract: Blastomycosis is a systemic disease caused by Blastomyces spp. fungi. To determine its epidemiology in blastomycosis-endemic Minnesota, USA, we evaluated all cases reported to public health officials during 1999-2018. We focused on time to diagnosis, exposure activities, and exposure location. A total of 671 cases and a median of 34 cases/year were reported. Median time to diagnosis was 31 days; 61% of patients were not tested for blastomycosis until they were hospitalized. The case-fatality rate was 10%, and … Show more

“…However, the drug of choice will depend on the drug susceptibility report of the isolate. In our study, 83% of patients received echinocandins, while 13% of patients received azoles as per sensitivity report; meanwhile, in a study by Arensman et al, 92% of patients received echinocandins and 8% received azoles [ 21 ]. There is currently no evidence to support combination therapy in bloodstream infections with this organism, although if the urinary tract or CNS is involved dual therapy may be necessary [ 22 ].…”

“…Data about environmental exposures, immunocompromised status, body site of infection, occupation, illness duration, and treatment were not available consistently from every state. Wisconsin and Minnesota conducted extensive follow-up on cases ( 19 , 25 ), providing deeper insight into state-specific features of blastomycosis. Collecting these types of data in a standardized way in additional states could help identify high-risk populations and activities and help inform prevention efforts.…”

“…We observed an overall mortality rate of 16.2% in this study cohort, which is higher than the mortality rate observed in recently published large cohorts. A US nationwide study found an overall in-hospital mortality rate of 6.9% (18), and an overall case-fatality rate in a cohort of 671 cases in Minnesota during 1999-2018 was 10% (19). In a systematic review and meta-analysis, the mortality estimate for general clinical cases of blastomycosis was 6.6% overall (20); substantial heterogeneity between the included studies was mainly attributed to inconsistent definitions of mortality.…”
