Bladder preservation therapy for muscle invasive bladder cancer: the past, present and future

Abstract: Radical cystectomy is the gold standard treatment for muscle invasive bladder cancer, but some patients have medically inoperable disease or refuse cystectomy to preserve their bladder function. Bladder preservation therapy with transurethral resection of the bladder tumor and concurrent chemoradiotherapy, known as trimodal treatment, is regarded to be a curative-intent alternative to radical cystectomy for patients with muscle invasive bladder cancer during the past decade. After the development of immune che… Show more

“…In recent years, deep learning algorithms have continued to advance and develop, and have been transformed and upgraded on the basis of the original algorithms. The CNN, a representative product of its development, has reportedly reduced the feature recognition error rate to 3.5%, and it can be said to be a very promising algorithm [ 21 , 22 ].…”

“…Open radical cystectomy is the standard method for treating patients with bladder cancer, and its therapeutic effects are universally recognized, but the surgical procedure is significantly traumatic for the patient and not only the amount of bleeding but also the postoperative complications adversely affect the patient's recovery [ 13 ]. Therefore, patients need a longer period of time to recover after the completion of the surgery, coupled with the degeneration of physiological organ functions, decreased immunity, and more underlying diseases in elderly patients, who have relatively low tolerance for open surgery [ 14 ]. Therefore, not only the surgical risk is high but also the postoperative complication rate is higher.…”

“…Approximately 400,000 new patients are diagnosed with bladder cancer worldwide each year, especially men, and one-third of them will die (Lenis et al, 2020;Richters et al, 2020;Teoh et al, 2020). The current treatment for bladder cancer is mainly urethral cystectomy and chemotherapy, but the therapeutic effect is effective and the prognosis is poor (Kimura et al, 2020;Tran et al, 2021). Similar to other tumors, the occurrence and development of bladder cancer are also regulated by various RNAs (Li et al, 2020a).…”
