Objectives: To elucidate clinicopathological risk factors for intravesical recurrence (IVR) in patients undergoing nephroureterectomy for upper urinary tract urothelial carcinoma (UUT-UC). Methods: We identified a study population of 151 consecutive patients without previous or concurrent bladder cancer who underwent nephroureterectomy for UUT-UC. IVR was assessed in relation to tumor location, size, and multifocality, operation modality and time, stage, grade, lymphovascular invasion, regional lymph node metastasis, preoperative urinary cytology, and perioperative chemotherapy. The median follow-up time was 24 months. Results: Of 151 patients, 51 (34%) developed IVR after nephroureterectomy, and 50 (98%) of the patients presented with IVR within 2 years. Tumor multifocality and site (located in ureter) were determined as risk factors for IVR by univariate analysis. In a multivariate analysis, only tumor multifocality (relative risk: 4.024, p = 0.001) was an independent predictor of IVR. Ten-year cancer-specific survival rates for the patients with and without IVR were 68 and 52%, respectively (p = 0.06). Conclusions: Tumor multifocality is a significant risk factor in developing IVR after surgery for UUT-UC. These results indicate that despite most IVR occurring within 2 years of treatment, it is necessary to follow such patients more closely using cystoscopy. However, IVR is unlikely to indicate a poorer prognosis.

GirişMultipl primer kanserler (MPK) ilk olarak 1889 yılında Billroth tarafından farklı histolojide, farklı organda ve farklı zaman aralığında ortaya çıkan tümörler olarak tanımlanmıştır (1). Bu tanımlama sistemi Warren ve Gates (2) tarafından 1932 yılında modifiye edilerek günümüzde kullanılan şeklini almıştır. Güncel tanımlamaya göre multipl primer tümörlerde olması gereken üç özellik (a) her tümörün klinik ve histolojik özellikleriyle malign olduğunun ortaya konulması; (b) her birinin histolojik olarak farklı tümörler olması; (c) herhangi bir tümörün diğerinin nüksü ya da metastazı olmadığının gösterilmesi şeklinde belirlenmektedir.İlk primer tümör genellikle indeks tümör olarak da adlandırılır. Çoklu primer tümörler senkron veya metakron tümörler olmak üzere iki grupta değerlendirilmektedir. Ancak senkron ve metakron tümör tanımı için bir uzlaşı söz konusu değildir. SEER (Surveillance, Epidemiology, and End Results) ölçütlerine göre ilk tümör tanısından itibaren iki ay içerisinde yeni tanı konmuş kanserler için senkron, iki aydan sonrası için metakron tanımının kullanılması önerilmektedir (3). İki aydan daha kısa sürede ortaya çıkan tümörler için bazı çalışmalarda simültane tümör terimi de kullanılmıştır (4). Moertel (5), yeni bir tanımlama getirerek ikincil tümör, indeks tümör tanısından itibaren altı ay içerisinde görülürse senkron, altı aydan sonra The improvements in cancer treatment prolonged survival in patients. Despite this survival benefit, chemotherapies, radiotherapies or combination therapies, and continuing exposure to the same carcinogenic agents may lead to secondary cancers. Multiple primary neoplasm is described as multiple tumors in a single patient posing distinct individual malignant characteristics with definite exclusion of one tumor is the metastasis of the other. According to the time of onset, these are considered to be synchronous or metachronous tumors. While synchronous tumors often occur due to carcinogen exposure, metachronous tumors often develop after treatments such as radiotherapy. Although the cause and developmental mechanisms of multiple primary tumors are not clear, several factors including immune deficiency, genetic instability, increased use of systemic chemotherapy and radiotherapy, increased survival, elderliness, and smoking have been implicated. The two developmental hypotheses in development of multiple primary tumors appear as field cancerization and common clonal origin. Multiple primary tumors often involve respiratory, gastrointestinal, and genitourinary systems. Transitional cell carcinoma of the urinary bladder may also rise as part of synchronous or metachronous multiple tumors. We still lack large scale studies relevant to the treatment of multiple primary cancers. Close follow-up in primary malignant tumor patients is of extreme importance for the risk of secondary cancers. Differential expression profile analysis of lncRNA UCA1α regulated mRNAs in bladder cancer

Urothelial carcinoma associated 1α (UCA1α) is a novel long non-coding RNA (lncRNA) that regulates bladder cancer proliferation, migration, and invasion. The target genes of UCA1α have, however, not been identified. To address this, a pCDNA3.1(+)-UCA1α over-expression vector was transfected into UM-UC-2 bladder cancer cells. Genes differentially expressed between pCDNA3.1(+)-UCA1α and pCDNA3.1(+) transfected cell were then detected by microarray and bioinformatics analysis. A total of 71 differentially expressed genes were identified, including 52 up-regulated genes and 19 down-regulated genes. As expected, the lncRNA UCA1α expression level was significantly increased when compared to that of pCDNA3.1(+) transfected cells. The five most significantly up-regulated and five most significantly down-regulated genes were selected, and their expression levels were also assessed by real time quantitative polymerase chain reaction and Western blot. The mRNA and protein expression levels of FOXI3 and GSTA3 were found to be significantly increased, and those of MED18 and TEX101 were found to be significantly decreased. Gene ontology (GO) clustering identified several significant biological processes, cellular components, and molecular functions, associated with lncRNA UCA1α over-expression. The differentially expressed genes were involved in several significant pathways as shown by Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway clustering. Cell proliferation activity was significantly increased following overexpression of lncRNA UCA1α increasing over culture time. The present study identifies, for the first time, potential target genes for lncRNA UCA1α in bladder cancer, and provides a significant reference for studying the role of lncRNA UCA1α in bladder cancer.
