Evaluate the lingual manifestations of COVID-19, and provide a clinical guide in managing these symptoms. Electronic databases, such as PubMed/Medline, and Scopus were searched until November 1, 2020, and only randomized controlled trials, cross-sectional and cohort studies, as well as case reports and series, and review articles in English were considered. A total of 40 studies were included in this study. Lingual involvement has been extensively reported in patients with coronavirus disease 2019 (COVID-19). The most common features of lingual involvements were red or light red, yellow coating, and greasy coating tongue, though other complications, such as pale, purple, white coating, grayish-black coating, rough, tender, puffy, spotty, prickles, fissured, and tooth-marked tongue was also reported. Poor oral hygiene, opportunistic infections (OIs), medications, and hyper-inflammatory response to infection are the most common predisposing factors for the onset of oral lesions in patients with COVID-19. In conclusion, the current review described the lingual manifestations of COVID-19, and as oral complaints are relatively common in COVID-19 patients, an intraoral examination should be conducted in all suspected cases of SARS-CoV-2 infection.

Introdução: A língua pilosa preta caracteriza-se pelo alongamento e pela pigmentação das papilas gustativas, conferindo-lhes uma aparência semelhante a pelos. Trata-se de uma doença benigna rara cujo diagnóstico é clínico, não havendo a necessidade de exames complementares de diagnóstico. Descrição do caso: Relata-se um caso de um homem de 34 anos, fumador, que recorreu à consulta aberta com queixas de alteração da cor da língua, disgeusia, glossodinia e halitose após realização do tratamento antibiótico para a erradicação da Helicobacter Pylori. À inspeção da cavidade oral observavam-se projeções semelhantes a pelos no dorso da língua com coloração esverdeada, não removível à raspagem. Pelas características clínicas foi considerada a patologia da língua pilosa preta, com sobreinfeção fúngica, como diagnóstico provável. Após a aplicação do antifúngico tópico nistatina, o reforço de uma higiene oral adequada e a diminuição do consumo de tabaco verificou-se a resolução clínica completa em cerca de 14 dias. Comentário: No presente caso expõe-se uma patologia da mucosa oral pouco frequente na prática da medicina geral e familiar, onde a anamnese e o exame objetivo são a chave para o seu diagnóstico. O caso relatado permite corroborar possíveis etiologias da língua pilosa preta descritas na literatura científica, nomeadamente a toma do antibiótico amoxicilina e os hábitos tabágicos. Traduz-se, ainda, num caso de eficácia terapêutica da abordagem utilizada.
