Bk Virus-Associated Fatal Renal Failure Following Late-Onset Hemorrhagic Cystitis in an Unrelated Bone Marrow Transplantation

Abstract: The human polyomavirus BK (BKV)-associated hemorrhagic cystitis (HC) has been a frequent and, seldom life-threatening complication after bone marrow transplantation (BMT). The authors report a male with melodysplastic syndrome, who developed BKV-associated late-onset HC 12 days after HLA-matched unrelated BMT. His urine contained epithelial cells with intranuclear inclusion bodies suggestive of BKV infection and was positive for BKV in polymerase chain reaction. He did not respond to any treatment for HC. In a… Show more

“…However, in some cases, high attention is focused on the fatal outcome of urinary tract obstruction. 4,37 Despite maximum efforts, effective therapy of LOHC has not been established until now. Base management including hyperhydration, bladder irrigation and transfusional support are often ineffective.…”

“…Clinical syndromes vary from microscopic hematuria to macroscopic hematuria, dysuria, and flank pain with or without renal insufficiency. Both BK polyoma virus and adenovirus are known to cause hemorrhagic cystitis after HCT; however, it is not clear whether viremia and viruria lead to renal insufficiency (68,69) (Figure 4). In a report of 21 autopsied transplant patients …”

“…BKV initiates an asymptomatic, lifelong persistent infection in approximately 80 to 90% of healthy human adults (6,27,48). The reactivation of BKV occurs during immunosuppression following kidney or bone marrow transplantation (17,24). Transplant recipients receiving more intensive immunosuppressive regimens are at a higher risk of viral reactivation (3).…”
