Bizarre leiomyoma of the prostate

Abstract: Obstruction of the urethra, caused by a prostatic tumour necessitated prostatectomy in a 49-yr-old man. Histology revealed a moderately cellular and vascular tumour with marked cellular atypia. After a follow-up for three years, the results of both the clinical investigations and prostatic needle biopsy were negative. Thus the original opinion of malignant prostatic mesenchymal tumour was revised, resulting in the diagnosis of bizarre leiomyoma. Subsequently the smooth muscle cell origin and the benign nature … Show more

“…Microscopically, it is characterized by intersecting bundles not atypical, smooth muscle cells with uniform cigar-shaped and slightly vesicular nuclei. The spindle cell population is desmin, smooth muscle actin (SMA) and androgen receptor positive [8]. If there is also an increased mitotic activity and necrosis, it is a leiomyosarcoma.…”

“…86,87 The benign nature was established by long-term follow-up in all of them 5,86-88 and by ploidy analysis in one case. 86 Cases of epithelioid leiomyoma (leiomyoblastoma) were also reported. [89][90][91] A rare case of claimed malignant transformation of a leiomyoma into a leiomyosarcoma over a 15-month period was 449 reported in the German literature.…”

“…However, Gaudin et al 31 excluded Wang's seven cases from being considered atypical stromal hyperplasia and likened them to symplastic leiomyoma as reported by others. [86][87][88] The smooth muscle nature in many of the reported cases was established by immunohistochemistry 5 and electron microscopy. 86,87 The benign nature was established by long-term follow-up in all of them 5,86-88 and by ploidy analysis in one case.…”

“…Smooth muscle tumours in the prostate are suggested to arise from the smooth muscle elements of periglandular tissues, the prostatic capsule, or Müllerian duct remnants. Prostatic LM is rare, with <100 cases reported 31–50 . Most tumours occur in the sixth decade of life, and patients present with obstructive urinary symptoms.…”
