Bitemporal hemianopia; its unique binocular complexities and a novel remedy

Abstract: Bitemporal hemianopic visual field impairment frequently leads to binocular vision difficulties. Patients with bitemporal hemianopia with pre-existing exophoria complain of horizontal diplopia, sometimes combined with vertical deviation (with pre-existing hyperphoria). The symptoms are a result of the phoria decompensating into a tropia (hemi-slide) due to the lack of retinal correspondence between the remaining nasal fields of both eyes. We measured these effects using a dichoptic perimeter. We showed that al… Show more

“…Where the damage is permanent, however, patients are left with very little binocular function. There are very few studies in which stereoscopic depth perception has been systematically studied, although a recent study investigated the deficits in two patients with bitemporal hemianopia [ 78 ]. Under normal binocular viewing, mismatched images in the two eyes can stimulate fusional eye movements to align the images [ 79 ].…”

“…5 In some patients treatment with optical measures such as prism can be helpful to reduce the frequency of diplopia. 14 The association between abnormalities of binocular vision and congenital chiasmal syndromes has been well documented. In adults, loss of stereopsis can be a presenting feature of a chiasmal lesion.…”

“…Factors influencing the postoperative outcome as well as the complication rate include age [14,32,35], body mass index (BMI) [9], number of surgeries [15], and the surgical approach used [18,29,34] as well as tumor size and sinus suprasellar growth [16]. Typical vision changes and field defects include the bitemporal hemianopsia, leading to binocular vision difficulties [28]. The visual postoperative outcome and recovery are known to be favorable, although influencing factors are currently discussed such as preoperative deficits, tumor size und tumor location, age, duration of symptoms, und tumor recurrence [6,22,31].…”
