Bisphosphonates

Abstract: Bisphosphonates belong to a class of compounds similar to pyrophosphate. In these compounds the oxygen atom of the pyrophosphate is replaced by a carbon atom resulting in a P-C-P bond. They exert a potent inhibitory effect on osteoclasts and are therefore potent antiresorptive agents. They reduce bone turnover, increase bone mineral density, and decrease fracture risk both at the lumbar spine and the hip. Bisphosphonates have a high affinity for bone surfaces, where they accumulate, mainly at sites of bone rem… Show more

“…Alendronate is classified as a bisphosphonate drug, like other medications in this class alendronate inhibits bone resorption via action on osteoclasts or on osteoclast precursors. There are other antiresorptive agent used in treatment of osteoporosis like selective estrogen receptor modulators (SERMS) and calcitonin 3 .…”

“…Thus, it is recommended that food and drink other than water should not be taken for 2 hours before and 30 minutes after a dose correspondence: hyo-Kyung han college of Pharmacy, Dongguk University-seoul, Donggukro-32, Ilsan-Donggu, goyang 410-820, republic of Korea Tel +823 1961 5217 Fax +823 1961 5206 email hkhan@dongguk.edu of risedronate. 10 This dosing regimen is very inconvenient for patients, resulting in poor compliance and suboptimal clinical effectiveness of risedronate. In addition, risedronate belongs to Biopharmaceutics Classification System class III (high solubility/low permeability) 11 and its oral bioavailability is very low (1%).…”

“…Thus, patients should take it on an empty stomach with water and remain upright for at least 30 minutes after taking the drug. 10,45 Use of chitosan-coated liposomes may prevent the interaction with food because risedronate remains inside the liposomes. Further, chitosan-coated liposomes may reduce esophageal irritation by entrapping the drug inside the liposome, keeping it away from direct contact with the esophagus.…”

“…The most frequently reported side effect attributed to the use of bisphosphonates is upper GI irritation. To date, nonrandomized studies [10][11][12][13] and one randomized, placebo-controlled trial 14 have confirmed the benefit of daily therapy with oral bisphosphonates, including alendronate, in treating CF-related bone loss.…”

“…8,9 Bisphosphonates belong to a class of compounds that exert a significant inhibitory effect on osteoclasts and are therefore potent antiresorptive agents. 10 They reduce bone turnover, increase bone mineral density (BMD), and decrease fracture risk both at the lumbar spine and the hip. Due to their selectivity in action, they are usually not associated with systemic side effects or serious adverse events.…”
