Estudos de biomonitoração do sistema urinário, circulatório e tecidos indicam grande exposição ao Bisfenol A Resumo Bisfenol A (BPA) é um dos produtos quí-micos mais produzido em todo o mundo, e a exposição humana a ele é considerada onipresente. Assim, há preocupações de que a quantidade de BPA para o qual os seres humanos estão expostos podem causar efeitos adversos à saúde. Nós examinamos muitas possibilidades sobre o porquê estudos de biomonitorização e toxicocinética podem chegar a conclusões aparentemente conflitantes. Mais de 80 estudos publicados de biomonitorização humana que mediram a concentração de BPA em tecidos humanos, urina, sangue e outros fluidos, juntamente com dois estudos de toxicocinéti-ca do metabolismo humano BPA foram examinados. BPA não conjugado foi detectado no sangue (nonanogramas por mililitro gama), e BPA conjugado foi detectado na grande maioria das amostras de urina. Em contraste, estudos de toxico-cinética propuseram que os seres humanos não são internamente expostos ao BPA. Dados disponíveis de estudos de biomonitorização indicam que a população em geral está exposta ao BPA e em risco de exposição interna ao BPA não conjugado. Os dois estudos de toxicocinética, que sugeriram a exposição humana ao BPA é insignificante, têm deficiências significativas e estão diretamente refutados por outros estudos e, portanto não são confiáveis para fins de avaliação de risco. Palavras-chave Disruptor endócrino, Exposição humana, Modelo PBPK/PBTK, Gravidez, Avaliação de risco, Toxicocinética Abstract Bisphenol A (BPA) is one of the highest-volume chemicals produced worldwide, and human exposure to BPA is thought to be ubiquitous. Thus, there are concerns that the amount of BPA to which humans are exposed may cause adverse health effects. We examined many possibilities for why biomonitoring and toxicokinetic studies could come to seemingly conflicting conclusions. More than 80 published human biomonitoring studies that measured BPA concentrations in human tissues, urine, blood, and other fluids, along with two toxicokinetic studies of human BPA metabolism were examined. Unconjugated BPA was routinely detected in blood (in the nanograms per milliliter range), and conjugated BPA was routinely detected in the vast majority of urine samples (also in the nanograms per milliliter range). In stark contrast, toxicokinetic studies proposed that humans are not internally exposed to BPA. Available data from biomonitoring studies clearly indicate that the general population is exposed to BPA and is at risk from internal exposure to unconjugated BPA. The two toxicokinetic studies that suggested human BPA exposure is negligible have significant deficiencies, are directly contradicted by hypothesis-driven studies, and are therefore not reliable for risk assessment purposes.

With the reference to the position statements of the Endocrine Society, the Paediatric Endocrine Society, and the European Society of Paediatric Endocrinology, the Polish Society of Endocrinology points out the adverse health effects caused by endocrine disrupting chemicals (EDCs) commonly used in daily life as components of plastics, food containers, pharmaceuticals, and cosmetics. The statement is based on the alarming data about the increase of the prevalence of many endocrine disorders such as: cryptorchidism, precocious puberty in girls and boys, and hormone-dependent cancers (endometrium, breast, prostate). In our opinion, it is of human benefit to conduct epidemiological studies that will enable the estimation of the risk factors of exposure to EDCs and the probability of endocrine disorders. Increasing consumerism and the industrial boom has led to severe pollution of the environment with a corresponding negative impact on human health; thus, there is great necessity for the biomonitoring of EDCs in Poland. StreszczenieW nawiązaniu do opublikowanych ostatnio stanowisk Towarzystwa Endokrynologicznego (Endocrine Society), Towarzystwa Endokrynologii Pediatrycznej (Pediatric Endocrine Society) oraz Europejskiego Towarzystwa Endokrynologii Pediatrycznej (European Society of Pediatric Endocrinology), Polskie Towarzystwo Endokrynologiczne (PTE) zwraca uwagę na negatywne skutki zdrowotne spowodowane ekspozycją na związki endokrynnie czynne (EDC, endocrine disrupting chemicals) używane w przemyśle jako składowe plastików, opakowań żywności, leków oraz kosmetyków. Postępująca urbanizacja, industrializacja i konsumpcjonizm prowadzą do zwiększonego skażenia środowiska, które wpływa negatywnie na organizmy żywe, w tym również na ludzkie zdrowie. Obecnie znanych jest ponad 800 związków, które wykazują zdolność zaburzania czynności układu dokrewnego. Narastająca częstość występowania przypadków zaburzeń endokrynnych, zaburzeń rozwoju narządów płciowych, zaburzeń metabolicznych zwraca szczególną uwagę na potencjalną rolę czynników środowiskowych. Stanowisko PTE zostało sprowokowane alarmującymi danymi o wzroście częstości występowania: zaburzeń płodności, wnętrostwa, przedwczesnego dojrzewania płciowego dziewcząt i chłopców, nowotworów hormonozależnych, w tym raka endometrium, sutka, gruczołu krokowego, jajnika i jąder. W opinii autorów konieczne jest przeprowadzenie w Polsce reprezentatywnych, długoterminowych badań epidemiologicznych pozwalających na ocenę związku pomiędzy ekspozycją na EDC a występowaniem zaburzeń czynności gruczołów wydzielania wewnętrznego. Niezwykle ważne wydaje się stworzenie wielodyscyplinarnej grupy badawczej. Uzyskane wyniki po przedstawieniu przedstawicielom rządu, mogłyby umożliwić wprowadzenie stosownych rozporządzeń prawnych.

Il existe une relation entre exposition aux perturbateurs endocriniens (PE) et carcinogenèse animale. Cependant, les données épidémiologiques sont insuffisantes. Le bisphénol A (BPA) est un PE ubiquitaire présent dans l’eau potable. Ses dérivés chlorés (Clx-BPA) sont suspectés d’avoir une action PE 100 fois supérieure. L’objectif de ce travail était d’évaluer la faisabilité d’une étude d’exposition hydrique au BPA et aux Clx-BPA dans une population de patientes opérées du sein. L’étude a été menée au CHU de Poitiers auprès de trois populations de femmes opérées du sein classées selon la gravité de leur lésion. Trois façons d’évaluer l’exposition hydrique ont été explorées : dosage dans des matrices biologiques, dans l’eau du robinet et administration d’un questionnaire sociodémographique validé. Le critère de jugement principal était la concentration en composés dans l’eau ou les matrices biologiques et les quantités d’eau apportées par voie orale et cutanée selon le questionnaire. Dans l’eau de boisson analysée, le BPA a été quantifié chez la totalité des patientes (116 ± 162 ng∙L‑1). Les concentrations en Clx-BPA étaient de 1,85 ± 0,70 ng∙L‑1. Les concentrations urinaires en BPA étaient de 2,6 ng∙mL‑1 en préopératoire et 3,8 ± 5,5 ng∙mL‑1 en postopératoire, les Clx-BPA n’ayant pas été quantifiés. Dans le tissu adipeux mammaire, le BPA a été retrouvé à 1,265 ± 0,058 ng∙g‑1, le BPA et le Clx-BPA n’ayant été détectés qu’à deux reprises. Cette étude a montré la faisabilité des dosages du BPA et des Clx-BPA dans les matrices biologiques et l’eau du robinet. La mise en place d’une cohorte multicentrique permettra d’étudier la relation entre exposition à ces PE et cancer du sein.There is a relationship between exposure to endocrine disrupting chemicals (EDCs) and animal carcinogenesis. However, epidemiological data are insufficient. Bisphenol A (BPA) is a ubiquitous EDC present in drinking water. Its chlorinated derivatives (Clx-BPA) are suspected to have an ED action 100 times stronger than BPA itself. The aim of this study was to assess the feasibility of a trial about water exposure to BPA and Clx-BPA in a population of patients having breast surgery. The study was conducted at the University Hospital of Poitiers in three populations of women having breast surgery and classified according to the severity of their injury. Three methods to assess water exposure were explored: determination in biological matrices, in tap water and administration of a validated socio-demographic questionnaire for water exposure. The primary endpoint was the concentration of compounds in water or biological matrices and the amount of water provided by oral and dermal routes according to the questionnaire.
