Resumo ObjetivoAvaliar a relação entre o impacto ambiental decorrente da extração de carvão e sua repercussão na saúde reprodutiva de uma população, residente em pequenas cidades do Sul do Brasil, pela observação da freqüência de defeitos congênitos nos recém-nascidos. Métodos Foram monitoradas as freqüências de oito defeitos congênitos em 10.391 nascidos vivos, ocorridos no período de janeiro de 1985 a dezembro de 1995 em um hospital localizado no município de São Jerônimo, RS. Destes, foram selecionados oito maiores defeitos congênitos, e a análise de suas prevalências ao nascimento visou reduzir os possíveis erros-diagnósticos. As taxas de defeitos congênitos foram comparadas às de outros hospitais geograficamente próximos e às fornecidas pelo Estudo Colaborativo Latino-Americano de Malformações Congênitas (ECLAMC) para a América Latina. Resultados/ConclusõesAs freqüências observadas não diferiram das encontradas em hospitais regionais e no ECLAMC. O pequeno número de crianças avaliadas mostra apenas algumas tendências, concluindo serem necessários outros estudos mais profundos. Abstract Objective To assess the relationship between the environmental impact of coal mining and its effects in the reproductive health of populations living in small towns of Southern Brazil by studying the frequency of birth defects in newborns. Methods The assessment of potential reproductive hazards in the population was based on an evaluation of hospital records of 10,391 newborns within a 10-year-period (from 1985 to 1995). Eight major birth defects were selected and their frequencies at birth was analyzed and compared to observed frequencies registered by the Latin American Study of Congenital Malformation (ECLAMC). Results/ConclusionsThe results show no increase in the frequencies of the birth defects studied, and rule out the existence of an increased teratogenic risk in this region. It's noteworthy that the small number of children evaluated allows identifying tendencies that can only be confirmed in larger population studies.

Previous investigations have shown that women exposed to polychlorinated biphenyls (PCBs) are at increased risk of giving birth to an infant with low birth weight (< 2,500 g), and that this relationship is stronger for male than for female infants. We have tested the hypothesis that residents in a zip code that contains a PCB hazardous waste site or abuts a body of water contaminated with PCBs are at increased risk of giving birth to a low-birth-weight baby. We used the birth registry of the New York State Vital Statistics to identify all births between 1994 and 2000 in New York State except for New York City. This registry provides information on the infant, mother, and father together with the zip code of the mother's residence. The 865 state Superfund sites, the 86 National Priority List sites, and the six Areas of Concern in New York were characterized regarding whether or not they contain PCBs as a major contaminant. We identified 187 zip codes containing or abutting PCB-contaminated sites, and these zip codes were the residences of 24.5% of the 945,077 births. The birth weight in the PCB zip codes was on average 21.6 g less than in other zip codes (p < 0.001). Because there are many other risk factors for low birth weight, we have adjusted for these using a logistic regression model for these confounders. After adjusting for sex of the infant, mother's age, race, weight, height, education, income, marital status, and smoking, there was still a statistically significant 6% increased risk of giving birth to a male infant of low birth weight. These observations support the hypothesis that living in a zip code near a PCBcontaminated site poses a risk of exposure and giving birth to an infant of low birth weight.

1995 show that about 1.5 million people have been exposed to site-specific contaminants. At 10% or more of the sites that had either completed or potentially completed exposure pathways, 56 substances were identified. Of these, 19 are either known or anticipated human carcinogens, and 9 are associated with reproductive or endocrine-disrupting effects. In this paper we present important concerns regarding hazardous waste sites including the impact on human health, ecology, and quality of life. To address such human-health related issues, the ATSDR has established a mixtures program that consists of three components: trend analysis to identify combinations of chemicals of concern, experimental studies to identify data that would be useful in the development and implementation of predictive decision support methodologies, and development of assessment methodologies and guidance to provide health assessors with the tools to incorporate the evaluation of multiple-chemical exposure into site assessments.
