Bird Fancier's Lung: A State-of-the-Art Review

Abstract: Bird fancier's lung (BFL) resulting from avian antigen exposure is a very common form of hypersensitivity pneumonitis. Its pathogenesis is modified by genetic polymorphisms located within the major histocompatibility complex, and also by smoking, which may decrease serum antibody response to inhaled antigen. Acute, subacute, and chronic presentations of BFL are recognized, but often overlap clinically. Continued antigen exposure in the chronic phase portends a worse prognosis. Chronic bronchitis symptoms may b… Show more

“…VATS or open lung biopsies are more reliable but seldom necessary [11,15]. The characteristic histological triad of the following: a) interstitial infiltrates of lymphocytes and plasma cells, b) bronchiolitis obliterans, and c) tiny interstitial loosely-formed non-necrotising granulomas, is specific but is usually not found in chronic hypersensitivity pneumonitis [11].…”

“…[23][24][25] Acute and subacute HP are not relevant to our topic as neither produces a UIP-pattern, and these can be explored elsewhere. [26][27][28][29][30] The distinguishing clinical, radiological and histopathological features of CHrHP are discussed below and summarised in table 3.…”

“…Multivariate Hypersensitivity pneumonitis (HP) is a rare and complex interstitial lung disease (ILD) triggered by inhaled antigens, with multiple known causative agents. [1][2][3][4][5][6][7][8][9] Treatment is primarily aimed at identifying and avoiding the implicated antigen. 1,10,11 Identifying the off ending antigen, however, is achieved only in about 40% of cases.…”
