Excision of the accessory bipartite fragment is widely used, but its long-term outcome is not known. We evaluated the outcome after surgical excision of a symptomatic accessory bipartite or multipartite patella fragment in young adult men performing their compulsory military service and determined the incidence of painful bipartite patellae in this group of skeletally mature adults. We followed 25 of 32 patients for a minimum of 10 years (mean, 15 years; range, 10-22 years). The incidence of painful, surgically treated bipartite patella was 9.2 per 100,000 recruits. Patients' median age at surgery was 20 years. There were 19 superolateral and six lateral bipartite fragments. Other radiographic findings were rare. At followup, the Kujala score mean was 95 points (range, 75-100 points), and osteoarthrotic changes (Kellgren-Lawrence Grade 1) were seen in two knees. No reoperations related to bipartite patella occurred during the followup. Symptomatic bipartite patella is rare and does not seem primarily associated with anatomic deviations, but when incapacitating pain persists despite nonoperative treatment, surgical excision seems to yield reasonable functional outcome and quick recovery with no apparent adverse sequelae. Our data suggest there is no reason to avoid this technically undemanding procedure for treating persistent symptoms of bipartite patella in young adults.

International Journal of Case Reports and Images (IJCRI) is an international, peer reviewed, monthly, open access, online journal, publishing high-quality, articles in all areas of basic medical sciences and clinical specialties.Aim of IJCRI is to encourage the publication of new information by providing a platform for reporting of unique, unusual and rare cases which enhance understanding of disease process, its diagnosis, management and clinico-pathologic correlations. The occurrence of bipartite patellar disruption simultaneously with quadriceps tendon avulsion is an extremely rare injury. A literature review found three documented cases in which only two repaired both the patella and tendon. We present a case report outlining this unusual injury and propose a method of surgical treatment. Case Report: A 45-year-old male presented following a fall directly onto his knee, with no significant medical history or previous injury. He was diagnosed with both separation of a type III bipartite patella and a clinical suspicion of a quadriceps tendon rupture. Intra-operatively, we found a complete tear of the quadriceps tendon at its insertion to the patella with disruption of the synchondrosis and significant separation of the bipartite patella. The displaced fragment was reduced and held using two cannulated screws, the quadriceps tendon was reattached to the superior pole of the patella using two anchor sutures and the retinaculum repaired. Conclusion:We could find no documented cases of using both cannulated screws and anchor sutures as a fixation method for this injury. We recommend this technique as we found that it gave a stronger fixation and was associated with a good functional outcome for our patient.

ÖZBipartite patella -patella bipartita olarak da bilinen, genellikle patellanın insidental ve asemptomatik saptanan bir bulgusudur. Buna rağmen, genellikle adolesan hastalarda olmak üzere tekrarlayan spor aktivitelerini takiben ya da akut travma sonrası ön diz ağrısı ortaya çıkabilir. Semptomatik hastalarda tedavinin en önemli basamağı ağrı yaratan aktivitelerden kaçınmaktır. Çoğu hastada konservatif yaklaşımla sonuç alınır. Cerrahi tedavi yalnızca konservatif yaklaşımdan sonuç alınamazsa düşünülmelidir. Aksesuar fragmanın eksizyonu en çok tercih edilen cerrahi yöntem olup oldukça iyi sonuçlar bildirilmiştir. Diğer cerrahi seçenekler; lateral retinakuler gevşetme, vastus lateralis kası gevşetmesi ve ayrılmış fragmanın internal fiksasyonudur. Ağrılı bipartite patellada farklı tedavi yaklaşımlarının olası sonuçlarını anlamak için; patellofemoral eklem fonksiyonunu ve kuadriceps kas gücünü korumak gereklidir.Anahtar sözcükler: Patella, aksesuar kemik, varyasyon Patella Bipartita ABSTRACT Bipartite patella also known as patella bipartite, is a condition that is usually an asymptomatic and incidental finding of patella. Nevertheless, it can be a cause of anterior knee pain following acut trauma or a result of overuse or strenuous sports activity seen generally in adolesant patients. The usual treatment for symptomatic bipartite patella is to avoid painful activities. Most patients recover with nonsurgical treatment. Surgery should be considered when only nonsurgical treatment fails. Excision of the fragment is the most preferred surgical option which has good outcomes. Other surgical options are; lateral retinacular release, detachment of the vastus lateralis muscle and internal fixation of the separated fragment. Understanding the possible consequences of different treatment approaches to painful bipartite patella is necessary to preserve quadriceps muscle strength and patellofemoral joint function.
