Bioreconstructive Poly-L/D-Lactide Implant Compared With Swanson Prosthesis in Metacarpophalangeal Joint Arthroplasty in Rheumatoid Patients: a Randomized Clinical Trial

Abstract: It was hypothesized that the bioresorbable interposition implant might offer a viable alternative to conventional silicone implant arthroplasty in rheumatoid metacarpophalangeal joint destruction. A randomized clinical study was performed to compare a stemless poly-L/D-lactide copolymer 96 : 4 (PLDLA) implant with the Swanson silicone implant. Results in 52 patients (53 hands and 175 joints) at a mean follow-up of 2 years (minimum 1 year) showed that the improvement in clinical assessments was comparable in bo… Show more

“…Due to severe osteolysis, bone perforations and diverse soft tissue problems were encountered during revision surgery in our patients , a new silicone implant is not an ideal option in revision MCP arthroplasty. The PLDLA interposition arthroplasty aims to avoid the foreign body reaction , prosthesis wear or fracture complications associated with the use of silicone implant (10,12,13).…”

“…14-17 More recently, high level of evidence (level 1) has come from two randomized controlled trials (RCTs) that compared SMPA to other implants. 18,19 The sample sizes for these RCTs were small, ranging from 33 to 52 patients, whereas the retrospective studies ranged from 28 to 264 patients. Follow-up time varied by study type with retrospective studies having longer follow-up (average range 2 and 14 years) whereas prospective studies (average range 1-3 years) and RCT’s (range 1-2 years) had shorter follow-up.…”

“…Bioresorbable poly-L/D-lactide implants were recently developed for use in small joint arthroplasty. Although range of motion and pain relief are similar to Swanson implants (70), there is no risk of implant fracture, foreign body reaction, and periimplant osteolysis. These implants were designed to offer temporary support to guide soft tissue ingrowth.…”
