Biometric Analysis – A Reliable Indicator for Diagnosing Taurodontism using Panoramic Radiographs

Abstract: Thus, taurodontism was diagnosed on a biometric analysis, which was otherwise missed on a cursory observation. It is therefore necessary from the clinical point of view, to diagnose even the mildest form of taurodontism by using metric analysis rather than just relying on a visual radiographic assessment, as its occurrence has many clinical implications and a diagnostic importance.

“…Taurodontism of primary or permanent molars was found in all of the examined individuals using biometric analysis from panoramic radiographs. This method is superior and more sensitive than cursory examination, especially in cases of milder forms of taurodontism [23]. The most evident signs were found in the single individual in whom permanent molars were assessed.…”

“…Extraction of teeth presenting taurodontism is usually complicated because of the change in furcation and volume of the tooth [ 3 ] and could lead to a risk of mandibular fracture, especially in cases with tooth length extending to the basilar as in the current case. Iatrogenic mandibular fracture associated with tooth removal can be the most serious complication and can occur immediately during the procedure or later in the first 4 weeks, being mostly associated with removal of third molars [ 4 ].…”

“…Taurodontism is characterized by an enlarged pulp chamber with apical displacement of pulp floor and absence of cementoenamel junction constriction diagnosed by radiographic examination. This dental anomaly is caused by failure on invagination of the Hertwig epithelial sheet diaphragm at the appropriate horizontal level, leading to changes in tooth shape [ 1 3 ]. Fracture of the mandible during extraction is a rare but underestimated complication, and most cases are associated with the removal of voluminous or deeply impacted third molars [ 4 , 5 ].…”

“…Bizim çalışmamızda ise taurodontizm varlığı, Stifman ve Chanannel tarafından geliştiril-miş, literatürde panoramik röntgenler üzerinde yapılan çalışmalarda güvenirliliği gösterilmiş, radyografi üzerinde ölçüme dayalı objektif bir yöntem kullanılarak, subjektif değerlendirmeyle oluşacak hata olasılığı minimuma indirilmiştir. 11,17 Etnik farklılıklar da göz önünde bulundurulduğunda, ça-lışma sonuçlarının kıyaslanmasının tam olarak gerçeği yansıtmayacağı kanısındayız.…”

“…2 Pulpa odasının apikale doğru yer değiştirme miktarı göz önünde bulundurularak taurodontizm dört grupta sınıflandırılmıştır; 5,11,12 1) Hipo-taurodont; pulpa odası köklere doğru hafif şekilde genişlemiştir, 2) Mezo-taurodont; pulpa odası oldukça geniş-lemiş ve kökler kısalmıştır, 3) Hiper-taurodont; pulpa odası neredeyse apekse yaklaşmış ve konkav şekilli apeks üzerinde hemen hemen dört kadar kanal vardır, 4) Piramidal; pulpa odası genişleyerek baştan sona kök içerisini doldurmuştur ve oldukça geniş bir apikal foramen mevcuttur. 12 Taurodontizmin, çürük veya dental prosedür-ler sırasında pulpa ekspozunu artırdığı bildirilmektedir.…”
