Biomarkers of endothelial activation/dysfunction distinguish subgroups of Ugandan patients with sepsis and differing mortality risks

Abstract: Conflict of interest: KCK and WCL are inventors on patents related to the use of angiopoietin markers, entitled "Angiopoietin-1 and-2 biomarkers for infectious diseases that compromise endothelial integrity" (application WO2009059404) and "Biomarkers for early determination of a critical or life threatening response to illness and monitoring response to treatment" (application CA2769433).

“…Under physiological conditions, Tie2 is tonically activated by Angpt-1, a vasculoprotective protein secreted by peri -endothelial cells and platelets [81] . In human sepsis, its intrinsic antagonist called Angpt-2 is rapidly released from activated endothelium, competitively inhibits Tie2 and predicts mortality as a biomarker [80] , [82] , [83] . Using AFM and conventional intravital microscopy, we could show that Angpt-2 mediates breakdown of the eGC as well [19] .…”

“…5,6 "A dysregulated host response to infection" 7 is the consensus definition of sepsis, 7 but the pathophysiological basis of this immune dysregulation remains incompletely defined. Hallmark hostresponses characterising sepsis 8,9 are also found in lifethreatening HIVassociated tuberculosis disease, including con current inflammatory and immuno suppressive signalling, 4,10 coagulation and endothelial activation, 11,12 innate cell activation and dysfunction, 4,13 and…”

“…It appears that, even in a resource-limited setting such as rural sub-Sahara Africa, the pathophysiology and course of sepsis may depend more on other factors (e.g., age, type and severity of infection, premorbid conditions, and individual genotypes) than the time elapsed between the onset of symptoms and hospital admission. A study analysing biomarkers from patients with sepsis in Uganda identified possible subgroups of patients with distinct host-response profiles ( 28 ).…”
