Biomarkers as Predictors of Recurrence following Curative Resection for Pancreatic Ductal Adenocarcinoma: A Review

Abstract: Pancreatic ductal adenocarcinoma (PDA) is the fourth most common cancer causing death in the United States. Early tumor recurrence is an important contributor to the dismal prognosis. The availability of an accurate prognostic biomarker for predicting disease recurrence following curative resection will be beneficial for patient care. Most of the currently studied biomarkers remain in the investigational phase, with CA 19-9 being the only biomarker currently approved by the FDA. Herein, we review the utility o… Show more

“…7,9 In a subset of PC nodes ( n = 14), miRNA-10b was increased by >1.5-fold ( p < 0.05), but in the other PC nodes ( n = 16), it did not differ from chronic pancreatitis ( p = 0.795) (Fig. 1a).…”

“…However, the preoperative diagnostic and prognostic capability of CA19-9 is restricted due to marginal sensitivity and specificity and falsely elevated readings in the setting of biliary obstruction. 6,7 Other studies have attempted to identify additional tumor markers but none has proven to be superior to CA 19-9.…”

“…Application of NMR (nuclear magnetic resonance) for mentioned indication is also wider for its ability to display outlet systems, which is beneficial. PET (positron emission tomography) CT is not applied as standard method in case of primary examination; its benefit is seen at the time of dispensarization (www.pathologyoutlines.com/pancreas.html; Hruban et al, 2000;Go et al, 2005;Delpu et al, 2011;Bockhorn et al, 2014;Diener et al, 2014;Edderkaoui and Eibl, 2014;Krška, 2014b;Osayi et al, 2014;Strobel and Büchler, 2014).…”

“…Even for the group of patients with local, resectable PDAC, the 5-year overall survival is just 20% [3]. The majority of patients who underwent radical surgery develop early recurrence within 12-18 months [4]. Reasons for the poor prognosis of this neoplasia are manifold: absence of specific symptoms during development, resulting in late diagnosis; high metastatic potential; resistance to chemotherapy/radiotherapy and the absence of reliable biomarkers for population screening.…”
