“…Despite the degree of ill health associated with GWI, results of standard physical evaluation and clinical diagnostic tests were mostly normal, with only limited differences that distinguished GWI cases from healthy controls. This is consistent with earlier Gulf War veteran reports [ 8 , 46 , 47 ] and exemplifies a longstanding challenge associated with GWI for veterans and their healthcare providers. Symptoms, by definition, are patients’ own experiences as opposed to externally “objective” measures of disease and have thus far been the only consistent marker of GWI in affected veterans.…”

“…The difference in biological profiles between GWI and the controls has been shown to be more distinct and consistent under an exercise challenge [6]. At rest, GWI profiles are variable and sometimes present with inconsistent results [63,64]. Past biomarker/biosignature studies have not grouped GWI in terms of prior trauma; as such, the distinct GWI L and GWI H subgroups intermixed at rest would be expected to yield mixed and inconsistent results compared to controls as the proportion of each group varies.…”

“…Ideally, such measurements could also contribute to diagnosis, especially given the difficulties experienced by some veterans in being diagnosed with GWI in order to access appropriate care. The search for good biological biomarkers that can be used diagnostically for GWI is ongoing [101]. A biomarker is defined by the National Academy of Sciences as a "xenobiotically-induced variation in cellular or biochemical components or processes, structures, or function that is measurable in a biological system or sample" [102].…”
