OBJECTIVE: Conventional body composition methods may produce biased quanti®cation of fat and fat-free mass in obese subjects, due to possible violation of the assumption of constant (73%) tissue hydration. We used an assumption-free, graphical method for interpreting body weight variation in obesity using bioelectrical measurements. DESIGN: 540 obese subjects with body mass index (BMI) b 31 kgam 2 without apparent edema were compared to 726 healthy subjects with BMI`31 kgam 2 and to 50 renal patients with apparent edema. A subgroup of 48 obese subjects were evaluated again after weight loss (8.6 kg, 3 BMI units) following one-month energy restriction (5 MJad, 1200 kcalad). 32 obese uremic patients were evaluated before and after a dialysis session (3.2 kg¯uid removed). Direct measurements obtained from standard 50 kHz frequency bioelectrical impedance analyzer were used as impedance vectors in the Resistance-Reactance Graph. RESULTS: 1) Impedance vectors of obese subjects could be discriminated from those of edematous patients with 91% correct allocation; 2) A signi®cant lengthening of vectors was observed after¯uid loss of 3 kg in obese subjects; but 3) A body weight loss of about 9 kg after energy restriction was associated with no vector displacement. CONCLUSION: A different impedance vector pattern was associated with body weight loss in obesity due to¯uid removal (vector lengthening) versus an energy-restricted diet (no vector displacement).

Objective: To examine the impact of HIV coinfection, socioeconomic status (SES) and severity of tuberculosis (TB) on the body composition and anthropometric status of adults with pulmonary TB. Design: Cross-sectional study. Setting: Five TB clinics in Dar es Salaam, Tanzania. Subjects: A total of 2231 adult men and women diagnosed with pulmonary TB, prior to the initiation of anti-TB therapy. Methods: We compared the distribution of anthropometric characteristics including body mass index (BMI), mid-upper arm circumference (MUAC), triceps skin-fold (TSF), and arm muscle circumference (AMC) by HIV status, SES characteristics, and indicators of TB severity (bacillary density in sputum and Karnofsky performance score). Similar comparisons were carried out with body composition variables from bioelectrical impedance analysis and albumin concentrations, in a subsample of 731 subjects. Results: In multivariate analysis, HIV infection was significantly associated with lower MUAC and AMC in both men and women, but not with BMI or TSF. Compared to HIV-uninfected women, those who were HIV infected had lower body cell mass (BCM) (adjusted difference ¼ À0.85 kg, P ¼ 0.04), intracellular water (À0.68 l, P ¼ 0.04), and phase angle (À0.52, P ¼ 0.02). Albumin concentrations were significantly lower in both men and women infected with HIV. Among HIV-infected men, CD4 cell counts o200/mm 3 were related to lower intracellular water, BCM, fat-free mass and phase angle. Independent of HIV infection, BMI and MUAC were positively related to SES indicators and the Karnofsky performance score; and inversely related to bacillary density. Conclusions: HIV infection is associated with indicators of low lean body mass in adults with TB; socioeconomic factors and TB severity are important correlates of wasting, independent of HIV. Malnutrition is associated with morbidity and mortality in HIV infected individuals. Little research has been conducted to identify the roles that clinical, illicit drug use and socioeconomic characteristics play in the nutritional status of HIV-infected patients. This cross-sectional analysis included 562 HIV-infected participants enrolled in the Nutrition for Healthy Living study conducted in Boston, MA and Providence, RI. The relationship between body mass index (BMI) and several covariates (type of drug use, demographic, and clinical characteristics) were examined using linear regression.Overall, drug users had a lower BMI than non-drug users. The BMI of cocaine users was 1.4 kg/ m 2 less than that of patients who did not use any drugs, after adjusting for other covariates (p= 0.02). The BMI of participants who were over the age of 55 years was 2.0 kg/m 2 less than that of patients under the age of 35, and BMI increased by 0.3 kg/m 2 with each 100 cells/mm 3 increase in CD4 count. HAART use, adherence to HAART, energy intake, AIDS status, hepatitis B and hepatitis C co-infections, cigarette smoking and depression were not associated with BMI in the final model.In conclusion, BMI was lower in drug users than non-drug users, and was lowest in cocaine users. BMI was also directly associated with CD4 count and inversely related to age more than 55 years old. HIV infected cocaine users may be at higher risk of developing malnutrition, suggesting the need for anticipatory nutritional support.
